New research sheds light on potentially negative effects of cannabis

Coughing fits, anxiety and paranoia are three of the most common adverse reactions to cannabis, according to a recent study by Washington State University researchers.

The researchers surveyed more than 1,500 college students on the type and frequency of adverse reactions they had experienced while using cannabis for their study in the Journal of Cannabis Research. They also collected information on the students' demographics, personality traits, cannabis use patterns and motives for using the drug.

"There's been surprisingly little research on the prevalence or frequency of various adverse reactions to cannabis and almost no research trying to predict who is more likely to experience these types of adverse reactions," said Carrie Cuttler, assistant professor of psychology and an author on the paper. "With the legalization of cannabis in Washington and 10 other states, we thought it would be important to document some of this information so that more novice users would have a better sense of what types of adverse reactions they may experience if they use cannabis."

More than 50% of the study participants reported having experienced coughing fits, anxiety and/or paranoia while using cannabis. On the other end of the spectrum, the three least-common reported reactions were fainting/passing out, non-auditory/visual hallucinations and cold sweats.

The researchers found the most frequently occurring adverse reactions were coughing fits, chest/lung discomfort and body humming, which a subset of the study group reported occurring approximately 30-40% of the time they were using cannabis.

Panic attacks, fainting and vomiting were considered the most distressing of the 26 possible adverse reactions.

"It is worth noting even the most distressing reactions to cannabis were only rated between moderately' and quite distressing," Cuttler said. "This suggests cannabis users do not, in general, find acute adverse reactions to cannabis to be severely distressing."

The least distressing reactions were reported to be body humming, numbness and feeling off balance/unsteady, the researchers found.

The study showed less frequent users are more likely to report negative effects. Additionally, individuals who reported using cannabis to try to fit in with friends, displayed cannabis use disorder symptoms or had anxiety sensitivity -- a tendency to imagine the worse possible outcome -- were more likely to report adverse reactions as well as experiencing a greater amount of distress.

"Interestingly, we didn't find that quantity of use during a single session predicted very much in terms of whether or not a person was going to have a bad reaction," Cuttler said. "It was the people who smoke on a less frequent basis who tend to have these bad experiences more often."

Moving forward, Cuttler hopes the results of the study will be put to use by doctors, medical cannabis distributors and even bud tenders to give people a better idea of what could go wrong when they get high.

"When you get any other kind of medication, there will be a leaflet or a warning printed on the bottle about the drug's potential side effects," Cuttler said. "There really isn't very much out there on this for cannabis, and we think that it is important for people to have access to this kind of information."

Cannabis-based medicine may cut seizures in half for those with tough-to-treat epilepsy

Taking cannabidiol may cut seizures in half for some children and adults with Lennox-Gastaut syndrome (LGS), a severe form of epilepsy, according to new information released today from a large scale controlled clinical study that will be presented at the American Academy of Neurology's 69th Annual Meeting in Boston, April 22 to 28, 2017. Cannabidiol is a molecule from the cannabis plant that does not have the psychoactive properties that create a "high."

 

Nearly 40 percent of people with LGS, which starts in childhood, had at least a 50 percent reduction in drop seizures when taking a liquid form of cannabidiol compared to 15 percent taking a placebo.

 

When someone has a drop seizure, their muscle tone changes, causing them to collapse. Children and adults with LGS have multiple kinds of seizures, including drop seizures and tonic-clonic seizures, which involve loss of consciousness and full-body convulsions. The seizures are hard to control and usually do not respond well to medications. Intellectual development is usually impaired in people with LGS.

 

Although the drop seizures of LGS are often very brief, they frequently lead to injury and trips to the hospital emergency room, so any reduction in drop seizure frequency is a benefit.

 

"Our study found that cannabidiol shows great promise in that it may reduce seizures that are otherwise difficult to control," said study author Anup Patel, MD, of Nationwide Children's Hospital and The Ohio State University College of Medicine in Columbus and a member of the American Academy of Neurology.

 

For the randomized, double-blind, placebo-controlled study, researchers followed 225 people with an average age of 16 for 14 weeks. The participants had an average of 85 drop seizures per month, had already tried an average of six epilepsy drugs that did not work for them and were taking an average of three epilepsy drugs during the study.

 

Participants were given either a higher dose of 20 mg/kg daily cannabidiol, a lower dose of 10 mg/kg daily cannabidiol or placebo as an add-on to their current medications for 14 weeks.

 

Those taking the higher dose had a 42 percent reduction in drop seizures overall, and for 40 percent, their seizures were reduced by half or more.

 

Those taking the lower dose had a 37 percent reduction in drop seizures overall, and for 36 percent, seizures were reduced by half or more.

 

Those taking the placebo had a 17 percent reduction in drop seizures, and for 15 percent, seizures were reduced by half or more.

 

There were side effects for 94 percent of those taking the higher dose, 84 percent of those taking the lower dose and 72 percent of those taking placebo, but most side effects were reported as mild to moderate. The two most common were decreased appetite and sleepiness.

 

Those receiving cannabidiol were up to 2.6 times more likely to say their overall condition had improved than those receiving the placebo, with up to 66 percent reporting improvement compared to 44 percent of those receiving the placebo.

 

"Our results suggest that cannabidiol may be effective for those with Lennox-Gastaut syndrome in treating drop seizures," said Patel. "This is important because this kind of epilepsy is incredibly difficult to treat. While there were more side effects for those taking cannabidiol, they were mostly well-tolerated. I believe that it may become an important new treatment option for these patients."

 

There is currently a plan to submit a New Drug Application to the FDA later this year.

Regular marijuana use linked to more sex

Despite concerns among physicians and scientists that frequent marijuana use may impair sexual desire or performance, the opposite appears more likely to be the case, new research indicates.

 

The jury's still out on rock 'n' roll. But the link between sex and at least one drug, marijuana, has been confirmed.

 

A study by investigators at the Stanford University School of Medicine indicates that, despite concerns among physicians and scientists that frequent marijuana use may impair sexual desire or performance, the opposite appears more likely to be the case.

 

The findings, to be published online Oct. 27 in the Journal of Sexual Medicine, are based on an analysis of more than 50,000 Americans ages 25-45. And they're unambiguous.

 

"Frequent marijuana use doesn't seem to impair sexual motivation or performance. If anything, it's associated with increased coital frequency," said the study's senior author, Michael Eisenberg, MD, assistant professor of urology. The lead author is Andrew Sun, MD, a resident in urology.

 

Hint of a causal connection

 

The study does not establish a causal connection between marijuana use and sexual activity, Eisenberg noted. But the results hint at it, he added. "The overall trend we saw applied to people of both sexes and all races, ages, education levels, income groups and religions, every health status, whether they were married or single and whether or not they had kids."

 

The study is the first to examine the relationship between marijuana use and frequency of sexual intercourse at the population level in the United States.

 

"Marijuana use is very common, but its large-scale use and association with sexual frequency hasn't been studied much in a scientific way," Eisenberg said.

 

According to the National Institute on Drug Abuse, more than 20 million adult Americans are current marijuana users. With the drug's legalization for medical or recreational use in 29 states, that number is climbing. But despite marijuana's growing status as a recreational drug, its status as a procreational drug remains ambiguous: On one hand, there are reports of erectile dysfunction in heavy users, and rigorous studies have found reduced sperm counts in men who smoke it; on the other hand, experiments conducted in animal models and humans indicate that marijuana stimulates activity in brain regions involved in sexual arousal and activity.

 

Looking at survey responses

 

To arrive at an accurate determination of marijuana's effect on intercourse frequency, Eisenberg and Sun turned to the National Survey of Family Growth, sponsored by the federal Centers for Disease Control and Prevention. The survey, which provides data pertaining to family structures, sexual practices and childbearing, reflects the overall demographic features of the U.S. population. Originally conducted at regular intervals, the survey is now carried out on an annual basis. It explicitly queries respondents on how many times they've had intercourse with a member of the opposite sex in the past four weeks, and how frequently they've smoked marijuana over the past 12 months.

 

The investigators compiled answers to those questions for all years since 2002, when the survey first began collecting data on men as well as women. They included data from respondents ages 25-45 and excluded a small percentage (fewer than 3 percent) of respondents who had failed to answer one or more relevant questions.

 

In all, Eisenberg and Sun obtained data on 28,176 women averaging 29.9 years of age and 22,943 men whose average age was 29.5. They assessed these individuals' self-reported patterns of marijuana use over the previous year and their self-reported frequency of heterosexual intercourse over the previous four weeks.

 

Some 24.5 percent of men and 14.5 percent of women in the analysis reported having used marijuana, and there was a positive association between the frequency of marijuana use and the frequency of sexual intercourse. This relationship applied to both sexes: Women denying marijuana use in the past year, for example, had sex on average 6.0 times during the previous four weeks, whereas that number was 7.1 for daily pot users. Among men, the corresponding figure was 5.6 for nonusers and 6.9 for daily users.

 

In other words, pot users are having about 20 percent more sex than pot abstainers, Eisenberg noted.

 

Positive association is universal

 

Moreover, Eisenberg said, the positive association between marijuana use and coital frequency was independent of demographic, health, marital or parental status.

 

In addition, the trend remained even after accounting for subjects' use of other drugs, such as cocaine or alcohol. This, Eisenberg said, suggests that marijuana's positive correlation with sexual activity doesn't merely reflect some general tendency of less-inhibited types, who may be more inclined to use drugs, to also be more likely to have sex. In addition, coital frequency rose steadily with increasing marijuana use, a dose-dependent relationship supporting a possible active role for marijuana in fostering sexual activity.

 

Nevertheless, Eisenberg cautioned, the study shouldn't be misinterpreted as having proven a causal link. "It doesn't say if you smoke more marijuana, you'll have more sex," he said.
