Increased attention to sad faces predicts depression risk in teenagers

Teenagers who tend to pay more attention to sad faces are more likely to develop depression, but specifically within the context of stress, according to new research from Binghamton University, State University of New York.

Researchers at Binghamton University, led by graduate student Cope Feurer and Professor of Psychology Brandon Gibb, aimed to examine whether attentional biases to emotional stimuli, assessed via eye tracking, serve as a marker of risk for depression for teenagers.

"Although previous studies from the lab have examined who is most likely to show biased attention to sad faces and whether attention to sad faces is associated with risk for depression, the current study is the first to look at whether these attention biases impact how teenagers respond to stress, both in the lab and in the real world," said Feurer.

Biased attention to sad faces is associated with depression in adults and is hypothesized to increase depression risk specifically in the presence, but not absence, of stress by modulating stress reactivity. However, few studies have tested this hypothesis, and no studies have examined the relation between attentional biases and stress reactivity during adolescence, despite evidence that this developmental window is marked by significant increases in stress and depression risk.

Seeking to address these limitations, the new study examined the impact of adolescents' sustained attention to facial displays of emotion on individual differences in both mood reactivity to real-world stress and physiological reactivity to a laboratory-based stressor. Consistent with vulnerability-stress models of attention, greater sustained attention to sad faces was associated with greater depressive reactions to real-world stress.

"If a teenager has a tendency to pay more attention to negative stimuli, then when they experience something stressful they are likely to have a less adaptive response to this stress and show greater increases in depressive symptoms," said Feurer. "For example, if two teenagers both have a fight with a friend and one teenager spends more time paying attention to negative stimuli (i.e., sad faces) than the other, then that teenager may show greater increases in depressive symptoms in response to the stressor, potentially because they are paying more attention to the stressor and how the stressor makes them feel."

The researchers believe that the biological mechanism behind this finding lies in the brain's ability to control emotional reactivity.

"Basically, if the brain has difficulty controlling how strongly a teenager responds to emotions, this makes it harder for them to look away from negative stimuli and their attention gets 'stuck,'" said Feurer. "So, when teenagers who tend to pay more attention to sad faces experience stress, they may respond more strongly to this stress, as they have difficulty disengaging their attention from negative emotions, leaving these teens at increased risk for depression."

"This is also why we believe that findings were stronger for older than younger adolescents. Specifically, the brain becomes more effective at controlling emotional reactivity as teens get older, so it may be that being able to look away from negative stimuli doesn't protect against the impact of stress until later adolescence."

There is increasing research showing that the way teenagers pay attention to emotional information can be modified through intervention, and that changing attention biases can reduce risk for depression. The current study highlights attention toward sad faces as a potential target for intervention, particularly among older teenagers, said Feurer.

The researchers recently submitted a grant that would let them look at how these attention biases change across childhood and adolescence.

"This will help us better understand how this risk factor develops and how it increases risk for depression in youth," said Gibb. "Hopefully, this will help us to develop interventions to identify risk for these types of biases so that they can be mitigated before they lead to depression."

Some children are more likely to suffer depression long after being bullied

Some young adults who were bullied as a child could have a greater risk of ongoing depression due to a mix of genetic and environmental factors according to a new study from the University of Bristol.

 

Researchers wanted to find out what factors influenced depression in young adults between the ages of 10 and 24 and why some people responded differently to risk factors such as bullying, maternal postnatal depression, early childhood anxiety and domestic violence.

 

Using detailed mood and feelings questionnaires and genetic information from 3,325 teenagers who are part of Bristol's Children of the 90s study, alongside evidence of these risk factors at nine points in time they found that childhood bullying was strongly associated with trajectories of depression that rise at an early age. Children who continued to show high depression into adulthood were also more likely to have genetic liability for depression and a mother with postnatal depression. However, Children who were bullied but did not have any genetic liability for depression showed much lower depressive symptoms as they become young adults.

 

University of Bristol PhD student Alex Kwong commented: "Although we know that depression can strike first during the teenage years we didn't know how risk factors influenced change over time. Thanks to the Children of the 90s study, we were able to examine at multiple time points the relationships between the strongest risk factors such as bullying and maternal depression, as well as factors such as genetic liability.

 

"It's important that we know if some children are more at risk of depression long after any childhood bullying has occurred. Our study found that young adults who were bullied as children were eight times more likely to experience depression that was limited to childhood. However, some children who were bullied showed greater patterns of depression that continued into adulthood and this group of children also showed genetic liability and family risk.

 

"However, just because an individual has genetic liability to depression does not mean they are destined to go on and have depression. There are a number of complex pathways that we still don't fully understand and need to investigate further.

 

"The next steps should continue to look at both genetic and environmental risk factors to help untangle this complex relationship that would eventually help influence prevention and coping strategies for our health and education services."

 

Lecturer in Psychiatric Epidemiology at the University of Bristol Dr Rebecca Pearson added: "The results can help us to identify which groups of children are most likely to suffer ongoing symptoms of depression into adulthood and which children will recover across adolescence. For example, the results suggest that children with multiple risk factors (including family history and bullying) should be targeted for early intervention but that when risk factors such as bullying occur insolation, symptoms of depression may be less likely to persist"

 

Karen Black, Chief Executive Officer for Bristol's Off the Record added: "At Off The Record we see a diverse mix of young people presenting with a range of needs, often depression and anxiety. Understanding some of the factors that influence this will further help us to shape services and our offer for young people. I would also hope that studies such as these will help change policy direction and spending so that we start to get upstream of the issues that we know affect mental health including education and family, prevention rather than cure ideally."

Teens who can describe negative emotions can stave off depression

Teenagers who can describe their negative emotions in precise and nuanced ways are better protected against depression than their peers who can't. That's the conclusion of a new study about negative emotion differentiation, or NED -- the ability to make fine-grained distinctions between negative emotions and apply precise labels -- published in the journal Emotion.

 

"Adolescents who use more granular terms such as 'I feel annoyed,' or 'I feel frustrated,' or 'I feel ashamed' -- instead of simply saying 'I feel bad' -- are better protected against developing increased depressive symptoms after experiencing a stressful life event," explains lead author Lisa Starr, an assistant professor of psychology at the University of Rochester.

 

Those who score low on negative emotion differentiation tend to describe their feelings in more general terms such as "bad" or "upset." As a result, they are less able to benefit from useful lessons encoded in their negative emotions, including the ability to develop coping strategies that could help them regulate how they feel.

 

"Emotions convey a lot of information. They communicate information about the person's motivational state, level of arousal, emotional valence, and appraisals of the threatening experience," says Starr. A person has to integrate all that information to figure out -- "am I feeling irritated," or "am I feeling angry, embarrassed, or some other emotion?"

 

Once you know that information you can use it to help determine the best course of action, explains Starr: "It's going to help me predict how my emotional experience will unfold, and how I can best regulate these emotions to make myself feel better."

 

The team found that a low NED strengthens the link between stressful life events and depression, leading to reduced psychological well-being.

 

By focusing exclusively on adolescence, which marks a time of heightened risk for depression, the study zeroed in on a gap in the research to date. Prior research suggests that during adolescence a person's NED plunges to its lowest point, compared to that of younger children or adults. It's exactly during this developmentally crucial time that depression rates climb steadily.

 

Previous research had shown that depression and low NED were related to each other, but the research designs of previous studies did not test whether a low NED temporally preceded depression. To the researchers, this phenomenon became the proverbial chicken-and-egg question: did those youth who showed signs of significant depressive symptoms have a naturally low NED, or was their NED low as a direct result of their feeling depressed?

 

The team, made up of Starr, Rachel Hershenberg, an assistant professor of psychiatry at Emory University, and Rochester graduate students Zoey Shaw, Irina Li, and Angela Santee, recruited 233 mid-adolescents in the greater Rochester area with an average age of nearly 16 (54 percent of them female) and conducted diagnostic interviews to evaluate the participants for depression.

 

Next, the teenagers reported their emotions four times daily over the period of seven days. One and a half years later, the team conducted follow-up interviews with the original participants (of whom 193 returned) to study longitudinal outcomes.

 

The researchers found that youth who are poor at differentiating their negative emotions are more susceptible to depressive symptoms following stressful life events. Conversely, those who display high NED are better at managing the emotional and behavioral aftermath of being exposed to stress, thereby reducing the likelihood of having negative emotions escalate into a clinically significant depression over time.

 

Depression ranks among the most challenging public health problems worldwide. As the most prevalent mental disorder, it not only causes recurring and difficult conditions for sufferers, but also costs the U.S. economy tens of billions of dollars each year and has been identified by the World Health Organization as the number one cause of global burden among industrialized nations. Particularly depression in adolescent girls is an important area to study, note the researchers, as this age brings a surge in depression rates, with a marked gender disparity that continues well into adulthood.

 

Adolescent depression disrupts social and emotional development, which can lead to a host of negative outcomes, including interpersonal problems, reduced productivity, poor physical health, and substance abuse. Moreover, people who get depressed during adolescence are more likely to become repeatedly depressed throughout their life span, says Starr. That's why mapping the emotional dynamics associated with depression is key to finding effective treatments.

 

"Basically you need to know the way you feel, in order to change the way you feel," says Starr. "I believe that NED could be modifiable, and I think it's something that could be directly addressed with treatment protocols that target NED."

 

The team's findings contribute to a growing body of research that tries to make inroads in the fight against rising rates of adolescent depression, suicidal thoughts, and suicide. According to the most recent CDC data, about 17 percent of high school students nationwide say they have thought of suicide, more than 13 percent said they actually made a suicide plan, and 7.4 percent attempted suicide in the past year.

 

"Our data suggests that if you are able to increase people's NED then you should be able to buffer them against stressful experiences and the depressogenic effect of stress," says Starr.

Early life stress plus overexpressed FKBP5 protein increases anxiety behavior

A new preclinical study adds to mounting evidence about the interplay between genetics and environment in mental health

A new preclinical study finds that anxiety-like behavior increases when early life adversity combines with high levels of FKBP5 -- a protein capable of modifying hormonal stress response. Moreover, the researchers demonstrate this genetic-early life stress interaction amplifies anxiety by selectively altering signaling of the enzyme AKT in the dorsal hippocampus, a portion of the brain primarily responsible for cognitive functions like learning and memory.

 

Researchers continue to dig for molecular clues to better understand how gene-environment interactions influence neuropsychiatric disease risk and resilience. An increasing number of studies point to a strong association between the FKBP5 gene and increased susceptibility to depression, anxiety, post-traumatic stress disorder and other mental health disorders.

 

Adding to the growing evidence, a new preclinical study by University of South Florida neuroscientists finds that anxiety-like behavior increases when early life adversity combines with high levels of FKBP5 -- a protein capable of modifying hormonal stress response. Moreover, the researchers demonstrate this genetic-early life stress interaction amplifies anxiety by selectively altering signaling of the enzyme AKT in the dorsal hippocampus, a portion of the brain primarily responsible for cognitive functions like learning and memory.

 

While more research is required, the study suggests that FKBP5 may be an effective target for treating anxiety and other mood disorders.

 

The findings were published June 4 in the International Journal of Molecular Sciences.

 

"We know that the combination of genetic variations and environmental factors can make people either more or less susceptible to mental illness -- even when they experience the same types of trauma," said senior author Laura Blair, PhD, assistant professor of molecular medicine at the USF Health Byrd Alzheimer's Center. Postdoctoral scholar Marangelie Criado-Marrero, PhD, was lead author of the study.

 

"We hypothesized that high FKBP5 and early life stress might yield neuropsychiatric symptoms through altered cellular stress response pathways in the brain."

 

In a series of experiments, newborn mice overexpressing human FKBP5 in the forebrain were divided into two groups -- one group was exposed to an early life stress (maternal separation), and the other was not. Two control groups were comprised of stressed and non-stressed mice without brain overexpression of FKBP5. At two months, when the mice were young adults, an elevated-plus maze with open and closed arms was used to test anxiety-like behavior. Compared to all other groups, the mice with high FKBP5 and early life stress showed more anxiety as measured by their tendency to stay within enclosed areas of the maze rather than naturally explore all arms of the maze.

 

The anxiety effect was more pronounced in the female mice than in males, an observation that aligns with sex differences noted in humans with anxiety disorders, Dr. Blair said.

 

The researchers also analyzed molecular changes in brains of the mice. They found that AKT signaling, specifically in the dorsal hippocampus, differed depending upon whether or not the mice with high FKBP5 had experienced maternal separation as newborns. AKT signaling -- shown to be altered in Alzheimer's disease and cancer as well as in mental health disorders -- affects brain cell survival and metabolism, and the brain's ability to adapt to new information.

 

"The AKT signaling pathway was inversely regulated as a result of early life stress. High FKBP5 normally decreases AKT signaling, but when early life stress was added to overexpressed FKBP5 that signaling activity increased," Dr. Blair said. "Overall, our findings highlight the importance of stress and genes (like FKBP5) in modulating vulnerability to mood disorders and learning impairments."

Depression among young teens linked to cannabis use at 18

Seattle-focused study suggests earlier intervention with depressed youths could reduce rate of cannabis-use disorder

A study looking at the cumulative effects of depression in youth, found that young people with chronic or severe forms of depression were at elevated risk for developing a problem with cannabis in later adolescence.

 

The study led by UW Medicine researchers interviewed 521 students recruited from four Seattle public middle schools. Researchers used data from annual assessments when students were ages 12-15 and then again when they were 18. The results were published in the journal Addiction.

 

"The findings suggest that if we can prevent or reduce chronic depression during early adolescence, we may reduce the prevalence of cannabis use disorder," said lead author Isaac Rhew, research assistant professor of psychiatry and behavioral sciences at the University of Washington School of Medicine.

 

What researchers called "a 1 standard deviation increase" in cumulative depression during early adolescence was associated with a 50 percent higher likelihood of cannabis-use disorder.

 

According to researchers, during the past decade cannabis has surpassed tobacco with respect to prevalence of use among adolescents. Cannabis and alcohol are the two most commonly used substances among youth in the United States. They pointed to one national study showing increases in prevalence of cannabis use disorder and alcohol use disorder in the United States, especially among young adults.

 

Longitudinal studies looking at the link between depression and later use of alcohol and cannabis, however, have been mixed. Some show a link. Others don't. But most studies have assessed adolescent depression at a single point in time -- not cumulatively, said the researchers. Further, there have been differences in how substance use has been measured ranging from the initiation of any use to heavier problematic forms of use.

 

The study oversampled for students with depressive and/or conduct problems. The researchers were surprised to see that the prevalence of cannabis and alcohol use disorder in this study was notably higher than national estimates with 21 percent meeting criteria for cannabis use disorder and 20 percent meeting criteria for alcohol use disorder at age 18.

 

What effect the easing of marijuana laws in Washington state had on the youth is unclear. Researchers said it would be informative to conduct a similar study in a state with more strict marijuana laws to understand whether the relationship between depression and cannabis misuse would still hold in areas where marijuana may be less accessible.

 

The age 18 substance abuse assessments occurred between 2007-2010. Washington state legalized medical cannabis in 1998 and its medical cannabis market expanded greatly after 2009 when the federal justice department issued a ruling known as the Ogden Memo. And in 2003, the city of Seattle made cannabis offenses the lowest enforcement priority for police and the city attorney.

Insomnia Linked to Depression in Young Adults

A new study confirms the persistent nature of insomnia and the increased risk of subsequent depression among individuals with insomnia.

 

According to the results, the annual prevalence of one-month insomnia increased gradually over time, with a cumulative prevalence rate of 20 percent and a greater than two-fold risk among women. In 40 percent of subjects, insomnia developed into more chronic forms over time. Insomnia either with or without comorbid depression was highly stable over time. Insomnia lasting two weeks or longer predicted major depressive episodes and major depressive disorder at subsequent interviews.

 

Seventeen to 50 percent of subjects with insomnia lasting two weeks or longer developed a major depressive episode reported in a later interview. "Pure" insomnia and "pure" depression were not longitudinally related to each other, whereas insomnia comorbid with depression was longitudinally related to both.

 

"We used to think that insomnia was most often just a symptom of depression. However, a growing body of evidence suggests that insomnia is not just a symptom of depression, but that it may actually precede depression. In other words, people who have insomnia but no depression are at increased risk for later developing depression. This study adds to our knowledge by including a much longer follow-up period than most previous studies," said Daniel J. Buysse, MD, of the University of Pittsburgh, lead author of the paper.

 

"We were also able to look separately at insomnia alone, depression alone, and combined insomnia-depression. The results show that insomnia seems to be followed by depression more consistently than the other way around. In addition, we found that insomnia tended to be a chronic problem that gets more persistent over time, whereas depression was a more intermittent problem."
