Health most common major stressful event in Americans' lives last year

A new poll released today that examines the role of stress in Americans' lives finds that about half of the public (49 percent) reported that they had a major stressful event or experience in the past year. Nearly half (43 percent) reported that the most stressful experiences related to health.

Bad effects on emotional well-being (63%) are the most common health effect reported by those with a great deal of stress in the last month, followed by problems with sleep (56%) and difficulty in thinking, concentrating, or making decisions (50%). About half of those with a great deal of stress as well as a chronic illness or disability say stress made the symptoms worse (53%) or made it harder for them to manage their chronic illness or disability (52%).

In addition, many report significant impacts from stress in other spheres of their lives. More than four in ten of those under a great deal of stress in the last month report that this stress made it harder to get along with family members (45%) and prevented them from spending time with family members (44%). Half of those who experienced a great deal of stress in the last month and are employed say stress made it harder to concentrate at work (51%), and 41% say it made it harder to take on extra responsibilities that could help advance their career.

Men's hot flashes: Hypnotic relaxation therapy may ease the discomfort

Men who experience hot flashes are unlikely to talk much about it, but they may find relief from their silent suffering if they are willing to try an unusual treatment, according to new research. After seven weeks of hypnotic relaxation therapy, a 69-year-old man who had uncontrolled hot flashes following prostate cancer surgery showed a drastic decrease not only in hot flashes but also an impressive improvement in sleep quality, according to the study.

The new research follows previous published studies by Elkins that found a marked decrease in hot flashes among postmenopausal women and also among breast cancer survivors who have undergone hypnotic relaxation therapy. It reduced hot flashes by as much as 80 percent, and research findings by clinically trained therapists show it also improved participants' quality of life and lessened anxiety and depression. (Quality of life included such issues as work, sexuality, social and leisure activities, mood and concentration.) "And that's all without the increased risk of breast cancer or heart disease associated with hormone treatments such as estrogen or progestin," Elkins said.

The weekly sessions of hypnosis of 187 women over a five-week period involved hypnosis by clinically trained therapists. The women also practiced self-hypnosis using audio recordings to visualize a snowy path or cool mountain creek. During the treatments, the women wore skin monitors with electrodes and kept diaries of when they had hot flashes, how often, how severe they were, and what might have triggered flashes, such as stress, spicy foods or being in a hot room.

Women also were encouraged to individualize their therapy. An example: One woman's imagery was walking to a lake from her porch to sit on lawn chairs with her family at sunset. Previous research has shown that people vary when it comes to being "hypnotizable" -- responsive to hypnosis -- but the consensus is that most people are at least moderately so. Besides having few or no side effects, hypnotic relaxation therapy is cost-saving and allows patients to be involved in their own healing.

Link between vitamin D, dementia risk confirmed

Vitamin D deficiency is associated with a substantially increased risk of dementia and Alzheimer's disease in older people, according to the most robust study of its kind ever conducted. An international team found that study participants who were severely vitamin D deficient were more than twice as likely to develop dementia and Alzheimer's disease.

Commenting on the study, Dr Doug Brown, Director of Research and Development at Alzheimer's Society said: "Shedding light on risk factors for dementia is one of the most important tasks facing today's health researchers. While earlier studies have suggested that a lack of the sunshine vitamin is linked to an increased risk of Alzheimer's disease, this study found that people with very low vitamin D levels were more than twice as likely to develop any kind of dementia.

"During this hottest of summers, hitting the beach for just 15 minutes of sunshine is enough to boost your vitamin D levels. However, we're not quite ready to say that sunlight or vitamin D supplements will reduce your risk of dementia. Large scale clinical trials are needed to determine whether increasing vitamin D levels in those with deficiencies can help prevent the dementia from developing."

Stress can result in the skin's nerve endings releasing an increased level of these chemicals. When this occurs, it can affect how and at what level our body responds to many important functions, such as sensation and control of blood flow, and can contribute to the symptoms of stress that we feel. Natural light in office boosts health

Office workers with more natural light exposure at the office had longer sleep duration, better sleep quality, more physical activity and better quality of life compared to office workers with less light exposure in the workplace, a study shows. "There is increasing evidence that exposure to light, during the day, particularly in the morning, is beneficial to your health via its effects on mood, alertness and metabolism," said the senior study author.

Employees with windows in the workplace received 173 percent more white light exposure during work hours and slept an average of 46 minutes more per night than employees who did not have the natural light exposure in the workplace. There also was a trend for workers in offices with windows to have more physical activity than those without windows.

Workers without windows reported poorer scores than their counterparts on quality of life measures related to physical problems and vitality, as well as poorer outcomes on measures of overall sleep quality and sleep disturbances.

"Light is the most important synchronizing agent for the brain and body," said Ivy Cheung, co-lead author and Ph.D. candidate in neuroscience in Zee's lab at Northwestern. "Proper synchronization of your internal biological rhythms with the earth's daily rotation has been shown to be essential for health." Also, people who get more light during the day may sleep better at night, which can also help improve health, Zee noted.

A study is shining new light on a sleep disorder called “sleep drunkenness.” The disorder may be as prevalent as affecting one in every seven people. Sleep drunkenness disorder involves confusion or inappropriate behavior, such as answering the phone instead of turning off the alarm, during or following arousals from sleep, either during the first part of the night or in the morning. An episode, often triggered by a forced awakening, may even cause violent behavior during sleep or amnesia of the episode.

Both long and short sleep times were associated with the sleep disorder. About 20 percent of those getting less than six hours of sleep per night and 15 percent of those getting at least nine hours experienced sleep drunkenness. People with sleep apnea also were more likely to have the disorder.

"These episodes of confused awakening have not gotten much attention, but given that they occur at a high rate in the general population, more research should be done on when they occur and whether they can be treated," said Ohayon. "People with sleep disorders or mental health issues should also be aware that they may be at greater risk of these episodes."

New research has found a way to identify those most susceptible to stress. That's a huge help for health-care professionals working to stop stress before it gets out of control. They found that those who exhibited a less variable heartbeat when they started worrying were more likely to be highly stressed later on, when faced with finals.

Gouin, who holds a Canada Research Chair in Chronic Stress and Health, explains: "At rest, a more variable heartbeat is a good thing. It shows that your parasympathetic nervous system is hard at work. That's the system that's responsible for the 'rest-and-digest' state of being -- the opposite of 'fight-or-flight.' The rest-and-digest phase puts you in a calm state that allows you to conserve and replenish your energy."

Depression increasing across the United States

Americans are more depressed now than they have been in decades, a recent study shows. Analyzing data from 6.9 million adolescents and adults from all over the country, researchers found that Americans now report more psychosomatic symptoms of depression, such as trouble sleeping and trouble concentrating, than their counterparts in the 1980s.

 

Compared to their 1980s counterparts, teens in the 2010s are 38 percent more likely to have trouble remembering, 74 percent more likely to have trouble sleeping and twice as likely to have seen a professional for mental health issues. College students surveyed were 50 percent more likely to say they feel overwhelmed, and adults were more likely to say their sleep was restless, they had poor appetite and everything was an effort -- all classic psychosomatic symptoms of depression.

Sleep duration affects risk for ulcerative colitis

Most Americans who spend part of the year on daylight saving time look forward to the extra hour of sleep when it’s time to “fall back” to standard time. We are a nation of sleep-deprived people, and experts say all ages suffer in various, unhealthy ways.

"For children, sleep deprivation can lead to behavior problems, trouble focusing and learning in school and it can affect their immune systems," said Dr. Aneesa Das, a sleep medicine specialist at Ohio State's Wexner Medical Center. "Chronic tiredness makes it harder to cope and process what's going on around you."

When children enter the teen years, sleep becomes a bigger issue. Das says a teen's circadian rhythm, or internal body clock, tells them to stay awake later and sleep later than children and adults do. She says only 15 percent of teenagers get the recommended sleep they need.

"Sleep is time the body uses to restore itself. Muscles and other tissues repair themselves, hormones that control growth, development and appetite are released. Energy is restored and memories are solidified, so we need to try to get regular sleep on a regular basis," Das said.

These daily cycles are run by a network of tiny, coordinated biological clocks. 1.    They're incredibly intricate. Biological clocks are composed of genes and proteins that operate in a feedback loop. Clock genes contain instructions for making clock proteins, whose levels rise and fall in a regular cyclic pattern. This pattern in turn regulates the activity of the genes. Many of the results from circadian rhythm research this year have uncovered more parts of the molecular machinery that fine-tune the clock.

2.    Every organism has them -- from algae to zebras. Many of the clock genes and proteins are similar across species, allowing researchers to make important findings about human circadian processes by studying the clock components of organisms like fruit flies, bread mold and plants.


3. Whether we're awake or asleep, our clocks keep ticking. While they might get temporarily thrown off by changes in light or temperature, our clocks usually can reset themselves.

4. Nearly everything about how our body works is tied to biological clocks. Our clocks influence alertness, hunger, metabolism, fertility, mood and other physiological conditions. For this reason, clock dysfunction is associated with various disorders, including insomnia, diabetes and depression. Even drug efficacy has been linked to our clocks: Studies have shown that some drugs might be more effective if given earlier in the day.

Long term shift work is linked to impaired brain power, finds research. The impact (for rotating shift patterns, at least) was stronger after a period of 10 or more years of exposure. And although the effects can be reversed, recovery may take at least five years, the findings suggest.

The second set of analyses looked at the impact of working a rotating shift pattern and found that compared with those who had never worked this type of shift, those who had, and had done so for 10 or more years, had lower global cognitive and memory scores -- equivalent to 6.5 years of age related cognitive decline.Finally, the researchers looked at whether stopping shift work was linked to a recovery in cognitive abilities.

The results indicated that it was possible to regain cognitive abilities after stopping shift work, but that this took at least five years, processing speeds excepted.

This is an observational study so no definitive conclusions can be drawn about cause and effect, but the disruption of the body clock as a result of shiftwork could generate physiological stressors, which may in turn affect the functioning of the brain, suggest the researchers.
Other research has also linked vitamin D deficiency caused by reduced exposure to daylight, to poorer cognition, they point out."The cognitive impairment observed in the present study may have important safety consequences not only for the individuals concerned, but also for society as a whole, given the increasing number of jobs in high hazard situations that are performed at night," warn the researchers.

At the very least the findings suggest that monitoring the health of people who have worked shift patterns for 10 years would be worth while, they say.

High rate of insomnia during early recovery from addiction

Insomnia is a "prevalent and persistent" problem for patients in the early phases of recovery from the disease of addiction — and may lead to an increased risk of relapse, according to a results of a recent study.

"Treating sleep disturbance in early recovery may have considerable impact on maintenance of sobriety and quality of life," according to Dr. Nicholas Rosenlicht of University of San Francisco and colleagues. They summarize the benefits of treatment, highlighting the role of effective behavioral approaches. The lead author was Katherine A. Kaplan, PhD, of Stanford University School of Medicine.

More generally, clinicians should be aware prescribing medications to treat insomnia may be "incongruent with or unpalatable to" treatment programs focusing on abstinence. In one survey, many addiction medicine specialists said they'd be reluctant to prescribe any medication to patients with sleep problems.

This belief has made behavioral approaches more widely used to treat patients with insomnia during recovery. In particular, evidence supports the use of cognitive-behavioral therapy (CBT). This multicomponent approach includes daily sleep diaries and questionnaires to gather information on the patient's insomnia and progress during treatment; and education on sleep and the effects of substances, including "sleep hygiene" practices to promote good sleep.

Moderate drinking is healthy only for some people

A new study confirms that moderate alcohol consumption can protect against coronary heart disease. But only for the 15% of the population that have a particular genotype.

The study included 618 Swedes with coronary heart disease and a control group of 3,000 healthy subjects. The subjects were assigned to various categories based on the amount of alcohol they consumed (ethanol intake). Meanwhile, they were tested in order to identify a particular genotype (CETP TaqIB) that previous studies had found to play a role in the health benefits of alcohol consumption.

Protective effect The results, which have been published in Alcohol, confirm the findings of the earlier studies. Moderate consumption of alcohol helps protect people with the genotype against coronary heart disease.

"In other words, moderate drinking has a protective effect among only 15% of the general population," says Professor Dag Thelle, Professor Emeritus at Sahlgrenska Academy, University of Gothenburg.

Sweeping advice Thus, the researchers believe that the advice frequently given about the health benefits of moderate alcohol consumption is far too sweeping.

"Moderate drinking alone does not have a strong protective effect," says Professor Lauren Lissner, who also participated in the study. "Nor does this particular genotype. But the combination of the two appears to significantly reduce the risk of coronary heart disease."

Anxiety can damage brain, research shows

People with mild cognitive impairment are at increased risk of converting to Alzheimer's disease within a few years, but a new study warns the risk increases significantly if they suffer from anxiety.

"Our findings suggest that clinicians should routinely screen for anxiety in people who have memory problems because anxiety signals that these people are at greater risk for developing Alzheimer's," said Dr. Linda Mah, principal investigator on the study, clinician-scientist with Baycrest's Rotman Research Institute, and assistant professor in the Department of Psychiatry at the University of Toronto. Dr. Mah is also a co-investigator in a multi-site study lead by the Centre for Addiction and Mental Health, and partially funded by federal dollars (Brain Canada), to prevent Alzheimer's in people with late-life depression or MCI who are at high risk for developing the progressive brain disease.

"While there is no published evidence to demonstrate whether drug treatments used in psychiatry for treating anxiety would be helpful in managing anxiety symptoms in people with mild cognitive impairment or in reducing their risk of conversion to Alzheimer's, we think that at the very least behavioural stress management programs could be recommended. In particular, there has been research on the use of mindfulness-based stress reduction in treating anxiety and other psychiatric symptoms in Alzheimer's --and this is showing promise," said Dr. Mah.

A new article suggests that major depression may result from parasitic, bacterial, or viral infection. Effect of once-daily, low-dose aspirin on cardiovascular death, other outcomes

Researchers examined whether once-daily, low-dose aspirin would reduce the total number of cardiovascular (CV) events (death from CV causes, nonfatal heart attack or stroke) compared with no aspirin in Japanese patients 60 years or older with hypertension, diabetes, or poor cholesterol or triglyceride levels.

The World Health Organization estimates that annual global mortality due to cardiovascular diseases (including heart attack and stroke) will approach 25 million by 2030. A recent study of trends in cardiovascular disease in Japan indicated that there has been, from 1960 to 2000, a steep increase in the prevalence of glucose intolerance, hypercholesterolemia, and obesity, probably due to the adoption of Western diets and lifestyles. By 2030, it is estimated that 32 percent of the Japanese population will be 65 years or older. Prevention of atherosclerotic cardiovascular diseases is an important public health priority in Japan due to an aging population, according to background information in the article.

This study included 14,464 patients (60 to 85 years of age) with hypertension, dyslipidemia (poor cholesterol or triglyceride levels), or diabetes mellitus who were randomized to aspirin (100 mg/d) or no aspirin in addition to ongoing medications. The patients were recruited by primary care physicians at 1,007 clinics in Japan. The study was terminated early by the data monitoring committee after a median follow-up of 5.02 years based on likely futility.
The researchers found that there was no statistically significant difference between the two groups in time to the primary end point (a composite of death from cardiovascular causes, nonfatal stroke, and nonfatal heart attack). At 5 years after randomization, the cumulative primary event rate was similar in participants in the aspirin group (2.77 percent) and those in the no aspirin group (2.96 percent).

Aspirin significantly reduced incidence of nonfatal heart attack and transient ischemic attack, and significantly increased the risk of extracranial hemorrhage requiring transfusion or hospitalization.

The authors write that despite inconsistent evidence for the benefit of aspirin in primary prevention of cardiovascular events, the benefits in secondary prevention are well documented, including in Japanese patients. "There is also a growing body of evidence to suggest benefits for aspirin in the prevention of colorectal and other cancers, and the prevention of cancer recurrence, including in the Japanese population. Reduction in the incidence of colorectal cancer may influence the overall benefit­risk profile of aspirin. Scientists have found that a particular set of cells in a small region of the brain are responsible for memory problems after sleep loss. By selectively increasing levels of a signaling molecule in these cells, the researchers prevented mice from having memory deficits.

In a new study, published in the Journal of Neuroscience, a team led by scientists from the University of Pennsylvania found that a particular set of cells in a small region of the brain are responsible for memory problems after sleep loss. By selectively increasing levels of a signaling molecule in these cells, the researchers prevented mice from having memory deficits.

"The challenge following this important study," Abel said, "was to determine if the impact of sleep deprivation was mediated by particular regions of the brain and particular neural circuits. We suspected that the hippocampus, the brain region that mediates spatial navigation and contextual memory, was critical."

"Thinking about people who do shift work or doctors who work long hours, if we can tackle the cognitive problems that result from sleep loss, that would be a great thing," Havekes said.
