Poor sleep in pregnancy can disrupt the immune system, cause birth complications

Poor sleep quality and quantity during pregnancy can disrupt normal immune processes and lead to lower birth weights and other complications, a new study finds. Women with depression also are more likely than non-depressed women to suffer from disturbed sleep and to experience immune system disruption and adverse pregnancy outcomes.
"Our results highlight the importance of identifying sleep problems in early pregnancy, especially in women experiencing depression, since sleep is a modifiable behavior," said Michele Okun, Ph.D., assistant professor of psychiatry at Pitt's School of Medicine and lead author of the report. "The earlier that sleep problems are identified, the sooner physicians can work with pregnant women to implement solutions."
Adequate and high-quality sleep, both in pregnant and non-pregnant women as well as men, is essential for a healthy immune system. Pregnancy often is associated with changes in sleep patterns, including shortened sleep, insomnia symptoms and poor sleep quality. These disturbances can exacerbate the body's inflammatory responses and cause an overproduction of cytokines, which act as signal molecules that communicate among immune cells.
"There is a dynamic relationship between sleep and immunity, and this study is the first to examine this relationship during pregnancy as opposed to postpartum," added Dr. Okun.
The findings reveal:
· Women with depression and poor sleep are at greatest risk for adverse birth-related outcomes. Cytokine levels may be one biological pathway through which this is accomplished, particularly with regard to preterm birth.
· Any shift in immunity, such as poor sleep and/or depression, could set the stage for increased risk for adverse outcomes.
· At 20 weeks, depressed pregnant women have higher levels of inflammatory cytokines compared to non-depressed women.
· At 30 weeks of pregnancy, differences in cytokines among depressed and non-depressed women were negligible, likely because as pregnancy progresses, levels of cytokines normally increase.

Link Between Stress and Increased Incidence of Psychosomatic Symptoms

In four out of ten cases, long-term stress suffered by women leads to some form of physical complaint. This is shown by a study of 1,500 women carried out at the Sahlgrenska Academy, University of Gothenburg, Sweden.
Among those women who reported stress, 40 percent had psychosomatic symptoms in the form of aches and pain in their muscles and joints, 28 percent suffered from headaches or migraines, and the same proportion reported gastrointestinal complaints.
"Even when the results have been adjusted for smoking, BMI and physical activity, we can see a clear link between perceived stress and an increased incidence of psychosomatic symptoms," says Dominique Hange, researcher at the Sahlgrenska Academy, University of Gothenburg.
Of those women who experienced long-term stress but who did not report any stress-related problems when the study began in 1968-69, 27 percent had new symptoms in the form of muscular and joint pain when they were followed up 12 years later, and around 15 percent experienced new complaints in the form of headaches and/or gastrointestinal problems.
"The most important conclusion is that single women, women who do not work outside the home and women who smoke are particularly vulnerable to stress. Here, we see a greater need for preventive measures from society."

Flame retardants, used in everyday products, may be toxic to children: Lower intelligence, hyperactivity seen

Chemicals called polybrominated diphenyl ethers have been used for decades to reduce fires in everyday products such as baby strollers, carpeting and electronics. A new study shows that prenatal exposure to the flame retardants is associated with lower intelligence and hyperactivity in early childhood
"In animal studies, PBDEs can disrupt thyroid hormone and cause hyperactivity and learning problems," said lead author Aimin Chen, MD, PhD, assistant professor in the Department of Environmental Health at University of Cincinnati College of Medicine. "Our study adds to several other human studies to highlight the need to reduce exposure to PBDEs in pregnant women."
"We found maternal exposure to PBDEs, a group of brominated flame retardants mostly withdrawn from the U.S. market in 2004, was associated with deficits in child cognition at age 5 years and hyperactivity at ages 2-5 years," Dr. Chen said. A 10-fold increase in maternal PBDEs was associated with about a 4 point IQ deficit in 5-year-old children.
Even though PBDEs, except Deca-BDEs, are not used as a flame retardant in the United States anymore, they are found on many consumer products bought several years ago. In addition, the chemicals are not easily biodegradable, so they remain in human tissues and are transferred to the developing fetus.
"Because PBDEs exist in the home and office environment as they are contained in old furniture, carpet pads, foams and electronics, the study raises further concern about their toxicity in developing children," Dr. Chen concluded.

Sustained Stress Heightens Risk of Miscarriage

Several studies have examined the impact of stress on a pregnancy -- both chronic stress, such as workload, and acute stress associated with traumatic events like the 9/11 terrorist attacks. They conclude that stress can lead to adverse birth outcomes, including miscarriage and premature birth.
Following the pregnancies of women from the Israeli town of Sderot, which is constantly under threat of rocket bombings from Gaza, and women from nearby Kiryat Gat, which is outside of Gaza's rocket range, the researchers demonstrated that those living under rocket fire were 59 percent more likely to miscarry than their neighbors.
Within the exposed group, the researchers also analyzed the intensity of exposure. Not every neighborhood in Sderot was subject to the same number of attacks, notes Prof. Lerner-Geva, and the researchers originally hypothesized that women in higher stress areas would have a higher probability of miscarriage. However, the results indicate that women in both high-intensity and low-intensity areas were at the same risk. One explanation is that the constant fear of attack is as stressful as the attacks themselves, she concludes.
Prevention through intervention
One advantage that healthcare providers have in dealing with populations under constant threat is that they can make use of early intervention, says Prof. Lerner-Geva. "Most of the Sderot pregnant women receive prenatal care through community health clinics. This presents an opportunity to run preventive interventions to reduce stress or even provide one-on-one counseling."

Stressful Life Events May Increase Stillbirth Risk

Pregnant women who experienced financial, emotional, or other personal stress in the year before their delivery had an increased chance of having a stillbirth, say researchers who conducted a National Institutes of Health network study.
Non-Hispanic black women were more likely to report experiencing stressful events than were non-Hispanic white women and Hispanic women. Black women also reported a greater number of stressful events than did their white and Hispanic counterparts. This finding may partly explain why black women have higher rates of stillbirth than non-Hispanic white or Hispanic women, the researchers said.
"We documented how significant stressors are highly prevalent in pregnant women's lives," said study co-author Marian Willinger, Ph.D., acting chief of the Pregnancy and Perinatology Branch of the Eunice Kennedy Shriver National Institute of Child Health and Human Development (NICHD), one of two NIH entities funding the research. "This reinforces the need for health care providers to ask expectant mothers about what is going on in their lives, monitor stressful life events and to offer support as part of prenatal care."

Postpartum Depression: Surprising Rate of Women Depressed After Baby

A surprisingly high number of women have postpartum depressive symptoms, according to a new, large-scale study by a Northwestern Medicine® researcher.
The study underscored the importance of prenatal as well as postpartum screening. Mothers' and infants' health and lives hang in the balance. The lives of several women who were suicidal when staff members called them for the screening were saved likely as a result of the study's screening and immediate intervention.
"In the U.S., the vast majority of postpartum women with depression are not identified or treated even though they are at higher risk for psychiatric disorders," said Northwestern Medicine lead study author Katherine L. Wisner, M.D. "It's a huge public health problem. A woman's mental health has a profound effect on fetal development as well as her child's physical and emotional development."
Maternal prenatal stress and depression is linked to preterm birth and low infant birth weight, which increases the risk of cardiovascular disease. Depression also affects a woman's appetite, nutrition and prenatal care and is associated with increased alcohol and drug use. Women with untreated depression have a higher body mass index preconception, which carries additional risks.
When a new mother is depressed, her emotional state can interfere with child development and increases the rate of insecure attachment and poor cognitive performance of her child, Wisner said.

Mom's Placenta Reflects Her Exposure to Stress and Impacts Offsprings' Brains

The mammalian placenta is more than just a filter through which nutrition and oxygen are passed from a mother to her unborn child. According to a new study by a research group from the University of Pennsylvania School of Veterinary Medicine, if a mother is exposed to stress during pregnancy, her placenta translates that experience to her fetus by altering levels of a protein that affects the developing brains of male and female offspring differently.
These findings suggest one way in which maternal-stress exposure may be linked to neurodevelopmental diseases such as autism and schizophrenia, which affect males more frequently or more severely than females.
"Most everything experienced by a woman during a pregnancy has to interact with the placenta in order to transmit to the fetus," said Tracy L. Bale, senior author on the paper and an associate professor in the Department of Animal Biology at Penn Vet. "Now we have a marker that appears to signal to the fetus that its mother has experienced stress."

Stress Hormone Foreshadows Postpartum Depression in New Mothers

Women who receive strong social support from their families during pregnancy appear to be protected from sharp increases in a particular stress hormone, making them less likely to develop postpartum depression, according to a new study published in Clinical Psychological Science, a journal of the Association for Psychological Science.
While pregnant women who felt strong support from their families and from the child's father had fewer depressive symptoms, there was no relationship between support from the father and levels of pCRH. Although father support was not as strong of a protective factor as family support in this study, "there is no doubt that fathers are a critical part of a healthy pregnancy," Hahn-Holbrook said. It could be that support from the father influences pCRH levels earlier in pregnancy, or father support may act by a different biological or behavioral pathway altogether, Hahn Holbrook said.
"Mothers with support from fathers may be more likely to practice healthy behaviors, which has been shown to contribute both to healthier babies, better birth outcomes, and lower postpartum disturbance," Dunkel Schetter added.
The study's results suggest that the timing of support interventions is especially important.
"Because levels of pCRH in the last trimester contributed to postpartum depression, early social support interventions might protect against both elevated pCRH and depressive symptoms," Dunkel Schetter said. "Too many interventions in the past have been mounted too late in pregnancy," she added.
More research should be conducted to determine when, what, and how to provide the optimal support to mothers during pregnancy, according to Dunkel Schetter. Her laboratory is conducting further research in this area.
Sharp increases in pCRH over the course of pregnancy are associated with preterm births, defined as births earlier than 37 weeks of gestation. Light Exposure During Pregnancy Key to Normal Eye Development

New research in Nature concludes the eye -- which depends on light to see -- also needs light to develop normally during pregnancy.
Scientists say the unexpected finding offers a new basic understanding of fetal eye development and ocular diseases caused by vascular disorders -- in particular one called retinopathy of prematurity that can blind premature infants. The research, led by scientists at Cincinnati Children's Hospital Medical Center and the University of California, San Francisco (UCSF), appears online Jan. 16 ahead of print publication.
"This fundamentally changes our understanding of how the retina develops," says study co-author Richard Lang, PhD, a researcher in the Division of Pediatric Ophthalmology at Cincinnati Children's Hospital Medical Center. "We have identified a light-response pathway that controls the number of retinal neurons. This has downstream effects on developing vasculature in the eye and is important because several major eye diseases are vascular diseases."
But researchers in the current study found that activation of the newly described light-response pathway must happen during pregnancy to activate the carefully choreographed program that produces a healthy eye. Specifically, they say it is important for a sufficient number of photons to enter the mother's body by late gestation, or about 16 days into a mouse pregnancy.

Impaired Melatonin Secretion May Play a Role in Premenstrual Syndrome

A new study by Douglas Mental Health University Institute researchers shows altered body rhythms of the hormone melatonin in Premenstrual dysphoric disorder (PMDD) women with insomnia. This finding may help explain some of the sleep disruptions experienced by women with PMDD, also known as premenstrual syndrome.
PMDD is a mood disorder which appears in the week preceding menses, and affects about 3-8% of women. PMDD sufferers can experience depression, tension, and irritability of sufficient intensity to interfere with daily activities and relationships. Disturbed sleep is also a common symptom of the disorder, with up to 70% of patients frequently reporting either poor sleep quality with increased awakenings or excessive sleepiness during the symptomatic phase.
The main finding was that compared to healthy controls, PMDD women had significantly decreased melatonin secretion levels during the night-time hours. PMDD women also had a further reduction of melatonin levels during their symptomatic luteal phase compared to the asymptomatic follicular phase.
Clinical implications of reduced melatonin in PMDD The prevalence of insomnia and depression are both about twice as high in women than in men, yet the reasons for this are still not fully understood. The current results highlight the importance of considering melatonin and circadian rhythms as factors leading to PMDD, with many clinical implications.

Vitamin D Tied to Women's Cognitive Performance

Two new studies appearing in the Journals of Gerontology Series A: Biological Sciences and Medical Sciences show that vitamin D may be a vital component for the cognitive health of women as they age.
Higher vitamin D dietary intake is associated with a lower risk of developing Alzheimer's disease, according to research conducted by a team led by Cedric Annweiler, MD, PhD, at the Angers University Hospital in France.
Very low levels of vitamin D (less than 10 nanograms per milliliter of blood serum) among older women were associated with higher odds of global cognitive impairment at baseline, and low vitamin D levels (less than 20 nanograms per milliliter) among cognitively-impaired women were associated with a higher risk of incident global cognitive decline, as measured by performance on the Mini-Mental State Examination
Similarly, investigators led by Yelena Slinin, MD, MS, at the VA Medical Center in Minneapolis found that low vitamin D levels among older women are associated with higher odds of global cognitive impairment and a higher risk of global cognitive decline.

Menopause: Relaxation Good Therapy for Hot Flushes

Women who have undergone group therapy and learned to relax have reduced their menopausal troubles by half, according to results of a study at Linköping University and Linköping University Hospital in Sweden.
Seven out of every ten women undergoing menopause have at some point experienced problems with hot flushes and sweating. For one in ten women, the problems lasted five years or longer, primarily causing discomfort in social situations and insomnia.
The situation triggered an interest in alternative forms of treatment. For her doctoral thesis, Women's Clinic consultant Elizabeth Nedstrand arranged a study where a group of women were randomly assigned to three different treatments alongside estrogen: acupuncture, exercise, and applied relaxation -- a method based on cognitive behaviour therapy developed by psychologist Lars-Göran Öst.
The results were so interesting that a larger randomised study around the effects of applied relaxation began n in 2007. 60 women who saw a doctor for moderate to severe symptoms occurring at least 50 times a week -- but who were otherwise completely healthy -- were randomly assigned to two groups: one had ten sessions of group therapy and the other received no treatment whatsoever. The results are now being published by Nedstrand and Lotta Lindh-Åstrand in the scientific journal Menopause.
Nedstrand herself conducted the therapy, which is based on learning to find the muscle groups in one's body and getting the body to relax with the help of breathing techniques.
The results were striking. The women in the treatment group reduced the number of hot flushes per day from an average of 9.1 to 4.4; the effect remained for three months after the last therapy session. The numbers in the control group also decreased, but only from 9.7 to 7.8.
The women in the therapy group also reported improved quality of life as regards memory and concentration, sleep, and anxiety. On the other hand, there were no statistically significant differences in stress hormone secretion.

Even moderate drinking in pregnancy can affect a child's IQ

Relatively small levels of exposure to alcohol while in the womb can influence a child's IQ, according to a new study using data from over 4,000 mothers and their children.
Current advice to pregnant women about moderate alcohol consumption during pregnancy is contradictory, with some official guidelines recommending complete abstinence and others suggesting that moderate use is safe. Previous studies have produced conflicting and inconsistent evidence on the effects of moderate alcohol intake on a child's IQ. This may be because it is difficult to separate the effects of moderate alcohol consumption from other lifestyle and social factors, such as smoking, diet, affluence, mother's age and education.
This study, believed to be the first substantial one of its kind, used genetic variation to investigate the effects of moderate (<1-6 units of alcohol per week) drinking during pregnancy among a large group of women and their children. Since the individual variations that people have in their DNA are not connected to lifestyle and social factors, the approach removes that potential complication.
Four genetic variants in alcohol-metabolising genes among the 4,167 children were strongly related to lower IQ at age eight. The child's IQ was on average almost two points lower per genetic modification they possessed.
But this effect was only seen among the children of women who were moderate drinkers. There was no effect evident among children whose mothers abstained during pregnancy, strongly suggesting that it was the exposure to alcohol in the womb that was leading to the difference in child IQ. Heavy drinkers were not included in the study.
Previous studies have relied on observational evidence, but this is problematic. Observational studies often find that moderate drinking is beneficial compared to abstention, but this is because mothers who drink in moderation during pregnancy are typically well educated, have a good diet and are unlikely to smoke -- all factors which are linked to higher IQ in the child, and which mask any negative effect that exposure to alcohol may have.
Speaking about the findings, the report's main author, Dr Sarah Lewis, said: 'Our results suggest that even at levels of alcohol consumption which are normally considered to be harmless, we can detect differences in childhood IQ, which are dependent on the ability of the fetus to clear this alcohol. This is evidence that even at these moderate levels, alcohol is influencing fetal brain development.'
Dr Ron Gray from the University of Oxford who led the research added: 'This is a complex study but the message is simple: even moderate amounts of alcohol during pregnancy can have an effect on future child intelligence. So women have good reason to choose to avoid alcohol when pregnant.'

Yoga reduces depression in pregnant women, boosts maternal bonding

University of Michigan study the first to show evidence that mindfulness yoga may offer effective treatment for depressed new mothers to be. Prenatal yoga may help women cope with depression.
Pregnant women who were identified as psychiatrically high risk and who participated in a 10-week mindfulness yoga intervention saw significant reductions in depressive symptoms, according to a University of Michigan Health System pilot feasibility study. Mothers-to-be also reported stronger attachment to their babies in the womb.
The findings were published in Complementary Therapies in Clinical Practice.
"We hear about pregnant women trying yoga to reduce stress but there's no data on how effective this method is," says lead author Maria Muzik, M.D., M.S., assistant professor of psychiatry and assistant research scientist at the Center for Human Growth and Development. "Our work provides promising first evidence that mindfulness yoga may be an effective alternative to pharmaceutical treatment for pregnant women showing signs of depression.
"This promotes both mother and baby wellbeing."
Mental health disorders during pregnancy, including depression and anxiety, have become a serious health concern. Hormonal changes, genetic predisposition and social factors set the stage for some expectant moms to experience persistent irritability, feelings of being overwhelmed and inability to cope with stress.
Untreated, these symptoms bear major health risks for both the mom and baby, including poor weight gain, preeclampsia, premature labor and trouble bonding with the new baby.
While antidepressants have proven to effectively treat these mood disorders, Muzik says, previous studies show that many pregnant women are reluctant to take these drugs out of concern for their infant's safety.

Men who do exercise produce better quality semen

A new study conducted by researchers in Spain links moderate physical activity in males with better hormone levels and sperm characteristics that favor reproduction compared to sedentary men.
Semen quality at large has dropped in the last 50 years. Amongst other factors, this is due to exposure to external agents and alcohol and tobacco consumption. This decline in sperm properties has caused an increase in reproductive problems.
Therefore, experts have studied the possible relationship between sperm quality and lifestyle habits in males. Published in the 'European Journal of Applied Physiology', the new study suggests that men who do moderate physical exercise have better hormone levels and their gonads undergo healthier spermatological processes.
Moderate exercise is the key
In 2010, the same researcher published a study showing that the sperm parameters of elite sportsmen (triathletes and waterpolo players) are worse than men who are just physically active. It is possible that the increased strain of training causes a decline in sperm quality.
"Despite that fact that more studies are needed to confirm these findings, we can suggest exercise to improve the hormonal environment and stimulate the sperm process," adds Vaamonde.

Marijuana use implicated in pregnancy problems

New research indicates marijuana-like compounds called endocannabinoids alter genes and biological signals critical to the formation of a normal placenta during pregnancy and may contribute to pregnancy complications like preeclampsia. A new study offers evidence that abnormal biological signaling by endocannabinoid lipid molecules produced by the body disrupts the movement of early embryonic cells important to a healthy pregnancy.
"The findings or our investigation raise concerns that exposure to cannabis products may adversely affect early embryo development that is then perpetuated later in pregnancy," said Sudhansu K. Dey, PhD., principal investigator on the study and division director. "Also, given that endocannabinoid signaling plays a key role in the central nervous system, it would be interesting in future studies to examine whether affected cell migration-related genes in early embryos also participate in neuronal cell migration during brain development."

Lack of Sleep Found to Be a New Risk Factor for Aggressive Breast Cancers

Lack of sleep is linked to more aggressive breast cancers, according to new findings published in the August issue of Breast Cancer Research and Treatment by physician-scientists from University Hospitals Case Medical Center's Seidman Cancer Center and Case Comprehensive Cancer Center at Case Western Reserve University.
"This is the first study to suggest that women who routinely sleep fewer hours may develop more aggressive breast cancers compared with women who sleep longer hours," said Dr. Thompson, who is Assistant Professor at Case Western Reserve University School of Medicine and lead author. "We found a strong correlation between fewer hours of sleep per night and worse recurrence scores, specifically in post-menopausal breast cancer patients. This suggests that lack of sufficient sleep may cause more aggressive tumors, but more research will need to be done to verify this finding and understand the causes of this association."
"Short sleep duration is a public health hazard leading not only to obesity, diabetes and heart disease, but also cancer," said Li Li, MD, PhD, a study co-author and family medicine physician in the Department of Family Medicine at UH Case Medical Center and Associate Professor of Family Medicine, Epidemiology and Biostatistics at Case Western Reserve University School of Medicine. "Effective intervention to increase duration of sleep and improve quality of sleep could be an under-appreciated avenue for reducing the risk of developing more aggressive breast cancers and recurrence."

How Well Is Depression in Women Being Diagnosed and Treated?

Major depression affects as many as 16% of reproductive-aged women in the U.S. Yet pregnant women have a higher rate of undiagnosed depression than nonpregnant women, according to a study published in Journal of Women's Health, a peer-reviewed publication from Mary Ann Liebert, Inc., publishers.
Jean Ko, PhD and coauthors from the Centers for Disease Control and Prevention (CDC), Atlanta, GA, found that more than 1 in 10 women ages 18-44 years had a major depressive event during the previous year -- representing about 1.2 million U.S. women -- but more than half of those women did not receive a diagnosis of depression and nearly half did not receive any mental health treatment. The article "Depression and Treatment among U.S. Pregnant and Nonpregnant Women of Reproductive Age, 2005-2009," further reports that disparities in receiving a diagnosis and treatment were associated with younger age, belonging to a racial/ethnic minority, and insurance status.

A pack of walnuts a day keeps the fertility specialist away?

After eating 75 grams of walnuts every day for 12 weeks, healthy young men aged 21 to 35 saw increased sperm vitality, motility, and morphology, researchers report.
Approximately 70 million couples experience subfertility or infertility worldwide, with 30 to 50 percent of these cases attributable to the male partner. Some studies have suggested that human semen quality has declined in industrialized nations, possibly due to pollution, poor lifestyle habits, and/or an increasingly Western-style diet.
The best sources of dietary PUFAs in a Western-style diet include fish and fish oil supplements, flax seed, and walnuts, the latter of which are rich sources of α-linolenic acid (ALA), a natural plant source of omega-3.
With support by the California Walnut Commission, Dr. Robbins' team selected 117 healthy men between the ages of 21 and 35 who ate a Western-style diet and split them into two groups: one (58 men) who would avoid eating tree nuts and another (59 men) who would eat 75 grams of walnuts per day. Previous studies had indicated that 75 grams of walnuts would be a dose at which blood lipid levels would change, but at which healthy young men would not gain weight.
Before the experiment began and then again 12 weeks later, the men's semen quality was analyzed according to conventional parameters of male fertility, including sperm concentration, vitality, motility, morphology, and chromosome abnormalities.
After 12 weeks, the team found no significant changes in body-mass index, body weight, or activity level in either group. The men consuming walnuts, however, had significantly increased levels of omega-6 and omega-3 (ALA) fatty acids and experienced improvement in sperm vitality, motility, and morphology. Those eating walnuts also had fewer chromosomal abnormalities in their sperm following the walnut dietary intervention. The control group, on the other hand, experienced no changes.
Although this research indicates that eating 75 grams of walnuts per day can positively affect a young man's sperm quality, it is still unknown whether the benefits would apply to young men with fertility problems and whether they would actually translate into increased fertility.

High Rates of Sleep Apnea in Women

New research from Umeå and Uppsala universities has found high rates of sleep apnea in women, despite the condition usually being regarded as a disorder predominantly of males.
The results found that obstructive sleep apnea was present in 50% of women aged 20-70 years. The researchers also found links between age, obesity and hypertension: 80% of women with hypertension and 84% of obese women suffered from sleep apnea. Additionally, severe sleep apnea was present in 31% of obese women aged 55-70 years old.
Lead author Dr. Karl Franklin said: "We were very surprised to find such a high occurrence of sleep apnea in women, as it is traditionally thought of as a male disorder. These findings suggest that clinicians should be particularly aware of the association between sleep apnea and obesity and hypertension, in order to identify patients who could also be suffering from the sleeping disorder."
