Meditation Therapy for Rheumatoid Arthritis Patients

A revered contemplative practice for centuries, meditation has recently inspired research into its therapeutic value for everything from anxiety disorders to heart attack prevention. A painful, progressive autoimmune disease, rheumatoid arthritis is associated with a high risk of depression -- double the risk of the healthy population, by conservative estimates -- and various forms of psychological distress. Increasingly, RA patients are turning to alternative therapies like meditation to ease the toll of their disease. Mindfulness-based stress reduction shows promise for easing psychological distress associated with disease symptoms.
Mindfulness-Based Stress Reduction (MBSR) is a meditation training program developed by Dr. Kabat-Zinn and colleagues at the University of Massachusetts Medical School. MBSR teaches participants to relate differently to thoughts and emotions, and continually focus the mind on the present moment to increase clarity and calmness. The program has been shown to improve psychological symptoms in patients with fibromyalgia, cancer, and multiple sclerosis, among other conditions.
Researchers with the University of Maryland School of Medicine set out to assess the effect of this meditation therapy on depressive symptoms, psychological distress, general well-being, and disease activity among RA patients. Featured in the journal Arthritis Care & Research, their study supports the potential benefits of prescribing a course in MBSR along with the conventional course of physical and pharmacological therapy.
"The study demonstrated that for patients with RA under routine medical supervision, an 8-week MBSR class plus a 4-month maintenance program had beneficial effects, and that it was safe and appealing to participants," notes investigator Elizabeth Pradhan, PhD. "For doctors wishing to offer patients a complement to medical management, mindfulness meditation may offer hope for improving psychological distress and strengthening well-being in patients with RA."

Researchers say that practicing even small doses of daily meditation may improve focus and performance. Even for those new to the practice, meditation enhanced performance and the ability to focus attention. Performance-based measures of cognitive function demonstrated improvements in a matter of weeks.
Meditation, according to Penn neuroscientist Amishi Jha and Michael Baime, director of Penn's Stress Management Program, is an active and effortful process that literally changes the way the brain works. Their study is the first to examine how meditation may modify the three subcomponents of attention, including the ability to prioritize and manage tasks and goals, the ability to voluntarily focus on specific information and the ability to stay alert to the environment.
In the Penn study, subjects were split into two categories. Those new to meditation, or "mindfulness training," took part in an eight-week course that included up to 30 minutes of daily meditation. The second group was more experienced with meditation and attended an intensive full-time, one-month retreat.
Researchers found that even for those new to the practice, meditation enhanced performance and the ability to focus attention. Performance-based measures of cognitive function demonstrated improvements in a matter of weeks. The study, to be published in the journal Cognitive, Affective, & Behavioral Neuroscience, suggests a new, non-medical means for improving focus and cognitive ability among disparate populations and has implications for workplace performance and learning.

Death rate for depressed heart patients double than for non-depressed heart patients

People who are diagnosed with coronary artery disease and then develop depression face a risk of death that's twice as high as heart patients without depression, according to a major new study.
The increased risk of death from any cause holds true whether the depression immediately follows the heart disease diagnosis or occurs even years later, according to Heidi May, PhD, a cardiovascular epidemiologist at Intermountain Medical Center Heart Institute and the study's lead author.
She said the findings point out the importance of screening for and treating depression even years after someone is diagnosed with heart disease.
The research, one of a number of studies to explore the connection between heart disease and development of depression by researchers at Intermountain Medical Center Heart Institute, will be published on July 28 in the European Heart Journal -- Quality of Care & Clinical Outcomes.
Researchers found that post-coronary artery disease depression was the single biggest predictor of death, and remained so even after researchers controlled for the other factors.
"No matter how long or how short it was, patients were found to have twice the risk of dying compared to those who didn't have a follow-up diagnosis of depression," Dr. May said. "Depression was the strongest risk factor for dying, compared to any other risk factors we evaluated. That included age, heart failure, diabetes, high blood pressure, kidney failure, or having a heart attack or stroke."
That association didn't change for patients who were previously diagnosed with depression before their heart disease diagnosis or for patients whose angiograms were performed for various reasons, which included stable angina, unstable angina, or heart attack.
Dr. May and the Intermountain Medical Center Heart Institute research team studied 24,138 patients who underwent angiographies, which determined they had coronary artery disease. To detect subsequent depression, the researchers looked at standardized diagnostic codes called International Classification of Diseases codes, or ICD codes.
Patients with depression were also placed into subcategories based on how long after their heart disease diagnosis the depression was identified.
Dr. May said most studies have looked at depression at a single point in time, such as within 30 days of a heart event or at the time of heart disease diagnosis. Just a handful of studies have looked over the course of a year, let alone years, such as this study, which followed patients for an average of 10 years after their coronary artery disease diagnosis to see if they were ever diagnosed with depression.
In all, 15 percent, or 2,646 patients, were diagnosed with depression at some point during follow-up. Of those, 27 percent were diagnosed within a year of their heart event, 24 percent between one and three years after, nearly 15 percent between three and five years after, and nearly 37 percent at least five years after a baseline heart disease event.
This study reinforces previous research investigating the link between depression, heart disease, and increased risks of death. It's already been shown that people with coronary artery disease don't live as long as their peers who don't have heart disease. And while life expectancy has increased with better therapies, surgeries, and more aggressive treatment of identified risk factors, depression has come under increasing scrutiny as a risk factor that could make a difference, if properly treated.
"We've completed several depression-related studies and been looking at this connection for many years," said Dr. May. "The data just keeps building on itself, showing that if you have heart disease and depression and it's not appropriately treated in a timely fashion, it's not a good thing for your long-term well-being."
Research has shown that the relationship is bi-directional: Depression may result in worse outcomes for people with heart disease, while the presence of heart disease may increase the likelihood that someone will develop depression.
Those with depression were significantly younger and more often female, diabetic, previously diagnosed with depression, and less likely to have presented with a heart attack compared to those who didn't have depression.
The study didn't explain the reason for the elevated risk of death, although Dr. May said one possibility is that depression impacts how closely patients follow their treatment plans.
"We know people with depression tend to be less compliant with medication on average and probably in general aren't following healthier diets or exercise regimens," she said. "They tend to do a poorer job of doing things that are prescribed than people without depression. That certainly doesn't mean you're depressed so you're going to be less compliant, but in general, they tend to follow those behaviors."
She also noted that physiological changes occur within the body when patients are diagnosed with depression, which might help explain the link.
The researchers emphasize the importance of continual screening of depression for all heart disease patients. "Patients who have depression need to be treated for it to improve not only their long-term risks but their quality of life," Dr. May said.
"I hope the takeaway is this: it doesn't matter how long it's been since the patient was diagnosed with coronary artery disease. Continued screening for depression needs to occur, said Dr. May. "After one year, it doesn't mean they're out of the woods. It should be ongoing, just like we keep measuring things like LDL cholesterol."

Omega-3 fatty acids fight inflammation via cannabinoids

Chemical compounds called cannabinoids are found in marijuana and also are produced naturally in the body from omega-3 fatty acids. A well-known cannabinoid in marijuana, tetrahydrocannabinol, is responsible for some of its euphoric effects, but it also has anti-inflammatory benefits. A new study in animal tissue reveals the cascade of chemical reactions that convert omega-3 fatty acids into cannabinoids that have anti-inflammatory benefits - but without the psychotropic high.
Foods such as meat, eggs, fish and nuts contain omega-3 and omega-6 fatty acids, which the body converts into endocannabinoids -- cannabinoids that the body produces naturally, said Aditi Das, a University of Illinois professor of comparative biosciences and biochemistry, who led the study. Cannabinoids in marijuana and endocannabinoids produced in the body can support the body's immune system and therefore are attractive targets for the development of anti-inflammatory therapeutics, she said.
In 1964, the Israeli chemist Raphael Mechoulam was the first to discover and isolate THC from marijuana. To test whether he had found the compound that produces euphoria, he dosed cake slices with 10 milligrams of pure THC and gave them to willing friends at a party. Their reactions, from nonstop laughter, to lethargy, to talkativeness, confirmed that THC was a psychotropic cannabinoid.
It wasn't until 1992 that researchers discovered endocannabinoids produced naturally in the body. Since then, several other endocannabinoids have been identified, but not all have known functions.
Cannabinoids bind to two types of cannabinoid receptors in the body -- one that is found predominantly in the nervous system and one in the immune system, Das said.
"Some cannabinoids, such as THC in marijuana or endocannabinoids can bind to these receptors and elicit anti-inflammatory and anti-pain action," she said.
"Our team discovered an enzymatic pathway that converts omega-3-derived endocannabinoids into more potent anti-inflammatory molecules that predominantly bind to the receptors found in the immune system," Das said. "This finding demonstrates how omega-3 fatty acids can produce some of the same medicinal qualities as marijuana, but without a psychotropic effect."

You're not yourself when you're sleepy

More than a third of Americans don’t get enough sleep, and growing evidence suggests it’s not only taking a toll on their physical health through heart disease, diabetes, stroke, and/or other conditions, but hurting their mental health as well.
According to a recent study led by Postdoctoral FellowIvan Vargas, PhD, in the journal Cognitive Therapy and Research, those who are sleep deprived lose some of their ability to be positive-minded people. That may not sound serious, but medical experts say an inability to think positively is a serious symptom of depression that could be dangerous if left unaddressed. An estimated 16.1 million adults experienced a major depressive episode in 2014.
"In general, we have a tendency to notice positive stimuli in our environment," said Vargas. "We tend to focus on positive things more than anything else, but now we're seeing that sleep deprivation may reverse that bias."
In their study, Vargas and his team took 40 healthy adults, and randomized them to either 28 consecutive hours awake, or a full eight hours of sleep. All participants participated in a computer test measuring their accuracy and response time at identifying happy, sad and neutral faces to assess how they pay attention to positive or negative information.
The team found that those who were acutely sleep deprived were less likely to focus on the happy faces. They didn't necessarily focus more on the negative, but were less likely to focus on the positive. The study may have implications for those experiencing depression and/or anxiety.
There are many symptoms of depression -- including feeling sad and no longer being able to enjoy things you typically would, but poor sleep is associated with a particularly serious sign of the condition.
"Depression is typically characterized as the tendency to think and feel more negatively or sad, but more than that, depression is associated with feeling less positive, less able to feel happy," Vargas says, "Similarly, if you don't get enough sleep, it reduces your ability to attend to positive things, which over time may confer risk for depression."
Interestingly enough, in the present study, those with a history of insomnia symptoms were less sensitive to the effects of the sleep loss. The authors believe this might be because those with a history of insomnia symptoms have more experience being in sleep-deprived conditions and have developed coping methods to modulate the effect of sleep loss.
Vargas and colleagues recently presented a related study at SLEEP 2017, the 31st Annual Meeting of the Associated Professional Sleep Societies LLC, on the association of insomnia and suicide, finding that people who suffer from insomnia are three times more likely to report thoughts of suicide and death during the past 30 days than those without the condition.
The study comes amid a growing body of knowledge associating sleep disorders and depression. For example, ongoing research presented this year at SLEEP 2017 from a multi-center NIH-sponsored "Treatment of Insomnia and Depression" study (abstract 0335 here) suggests that cognitive-behavioral therapy for insomnia (CBT-I) may help achieve depression remission in those suffering from both depression and insomnia who sleep at least 7 hours each night. (A clinical practice guideline published in 2016 in Annals of Internal Medicine recommends CBT-I (not sleep medications) as the initial treatment for chronic insomnia.
Additionally, a new study in the journal Child Development furthers our understanding of the connection between late night cell phone use, mental health, and disrupted sleep, finding that using a cell phone at night can increase depression in teenagers and lower their self-esteem.

Generous people live happier lives

Generosity makes people happier, even if they are only a little generous. People who act solely out of self-interest are less happy. Merely promising to be more generous is enough to trigger a change in our brains that makes us happier, neuroeconomists found in a recent study.
What some have been aware of for a long time, others find hard to believe: Those who are concerned about the well-being of their fellow human beings are happier than those who focus only on their own advancement. Doing something nice for another person gives many people a pleasant feeling that behavioral economists call a warm glow. In collaboration with international researchers, Philippe Tobler and Ernst Fehr from the Department of Economics at the University of Zurich investigated how brain areas communicate to produce this feeling. The results provide insight into the interplay between altruism and happiness.
Even a little generosity makes people happier
In their experiments, the researchers found that people who behaved generously were happier afterwards than those who behaved more selfishly. However, the amount of generosity did not influence the increase in contentment. "You don't need to become a self-sacrificing martyr to feel happier. Just being a little more generous will suffice," says Philippe Tobler.
Before the experiment started, some of the study participants had verbally committed to behaving generously towards other people. This group was willing to accept higher costs in order to do something nice for someone else. They also considered themselves happier after their generous behavior (but not beforehand) than the control group, who had committed to behaving generously toward themselves.
Intent alone suffices to cause neural changes
While the study participants were making their decision to behave or not to behave generously, the researchers examined activity in three areas of the participants' brains: in the temporoparietal junction (where prosocial behavior and generosity are processed), in the ventral striatum (which is associated with happiness), and in the orbitofrontal cortex (where we weigh the pros and cons during decision-making processes). These three brain areas interacted differently, depending on whether the study participants had committed to generosity or selfishness.
Simply promising to behave generously activated the altruistic area of the brain and intensified the interaction between this area and the area associated with happiness. "It is remarkable that intent alone generates a neural change before the action is actually implemented," says Tobler.
Benefit from the promise to behave generously
"Promising to behave generously could be used as a strategy to reinforce the desired behavior, on the one hand, and to feel happier, on the other," says Tobler. His co-author Soyoung Park adds: "There are still some open questions, such as: Can communication between these brain regions be trained and strengthened? If so, how? And, does the effect last when it is used deliberately, that is, if a person only behaves generously in order to feel happier?"
About the experiment
At the beginning of the experiment, the 50 participants were promised a sum of money that they would receive in the next few weeks and were supposed to spend. Half of the study participants committed to spending the money on someone they knew (experimental group, promise of generosity), while the other half committed to spending the money on themselves (control group).
Subsequently, all of the study participants made a series of decisions concerning generous behavior, namely, whether to giving somebody who is close to them a gift of money. The size of the gift and the cost thereof varied: One could, for example, give the other person five francs at a cost of two francs. Or give twenty francs at a cost of fifteen. While the study participants were making these decisions, the researchers measured activity in three brain areas: in the temporoparietal junction, where prosocial behavior and generosity are processed; in the ventral striatum, which is associated with happiness; and in the orbitofrontal cortex, where we weigh the pros and cons during decision-making processes. The participants were asked about their happiness before and after the experiment.

Controlled temperature change inside ear can prevent migraines

The application of gentle cooling and warming currents inside the ear canal can provide relief for migraine sufferers, new research has shown.
Volunteers in the study who had a history of migraines experienced a significant reduction in the number of migraines they normally experienced in a month after using a technique known as caloric vestibular stimulation (CVS).
CVS activates the balance organs which are believed to alter activity in the area of the brain, known as the brainstem, associated with the onset of migraine headaches.
Dr David Wilkinson, of the University's School of Psychology, helped lead the randomised, double-blinded, placebo-controlled trial. It was carried out across the US and UK, involving 81 volunteers with a history of between four and 14 migraine attacks per month.
The volunteers self-administered caloric vestibular stimulation daily for 20 minutes over a period of three months. The thermal currents were delivered by aluminium earpieces seated within padded headphones, powered and controlled by a small, hand-held device.
The findings demonstrated that the treatment reduced both the number of migraine days per month (the active treatment group experienced a reduction of 3.6 days compared to 0.9 days in the placebo group) as well as headache pain and the consequent need for migraine abortive prescription medications.
Dr Wilkinson said the results indicated that vestibular stimulation 'may address the existing need for new preventative therapies for episodic migraine'.
The findings were presented at the American Headache Society's annual meeting in June, where Professor Peter Goadsby, Chair of its Science Committee, said that 'many patients want non-drug options, so developing a non-drug therapy such as this may provide that.'
A second, expanded study will begin this summer, involving another collaboration between the University of Kent and US medical device company Scion Neurostim, who produced the CVS delivery device and will again fund the study, as well as local GP

The secret connection between anxiety, sleep

You may have experienced sleepless nights when you were anxious, stressed or too excited. Such emotions are well-known to affect wakefulness and can even cause insomnia, though the underlying mechanisms in our brain have still been unclear. Scientists have spotted neurons that play crucial roles in connecting emotions and sleep, shedding light on the future discovery of drug targets for anxiety disorder and/or sleep disorders.
Encountering predators, adapting to a novel environment or expecting a reward ― these stressful or emotionally-salient situations require animals to shift their behavior to a vigilant state, altering their physiological conditions through modulation of autonomic and endocrine functions.
The bed nucleus of the stria terminalis (BNST) is a part of the extended amygdala, which is generally considered as a key player in stress response, fear and anxiety. Through projections to various brain regions including relay nuclei of the autonomic nervous system, hypothalamic regions and the central nucleus of the amygdala, the BNST controls endocrine and autonomic reactions in response to emotionally-salient stimuli, along with behavioral expression of anxiety and fear.
A group of researchers led by Takeshi Sakurai, Vice Director of the International Institute for Integrative Sleep Medicine (WPI-IIIS), University of Tsukuba, found that acute optogenetic excitation of GABAergic neurons in BNST during non-rapid eye movement (NREM) sleep in mice resulted in immediate transition to a wakefulness state without the function of orexins, highly important neuropeptides for maintaining wakefulness. Notably, stimulation of the same neurons during REM sleep did not show any effects on sleep/wakefulness states.
Prolonged excitation of GABAergic neurons in BNST by a chemogenetic method evoked a longer-lasting, sustained wakefulness state, and it was abolished by administering a dual orexin receptor blocker (antagonist) DORA 22 in advance, meaning that orexins are involved in this phenomenon.
"Our study revealed a role of the BNST GABAergic system in sleep/wakefulness control, especially in shifting animals' behavioral states from NREM sleep to wakefulness. It also provides an important insight into the pathophysiology of insomnia and the role of orexin in arousal regulation, which will hopefully lead to the first step to develop remedies for sleep disorders," Sakurai says.

Mediterranean-style diets linked to better brain function in older adults

Eating foods included in two healthy diets -- the Mediterranean or the MIND diet -- is linked to a lower risk for memory difficulties in older adults, according to a new study.
The Mediterranean diet is rich in fruits, vegetables, whole grains, beans, potatoes, nuts, olive oil and fish. Processed foods, fried and fast foods, snack foods, red meat, poultry and whole-fat dairy foods are infrequently eaten on the Mediterranean diet.
The MIND diet is a version of the Mediterranean diet that includes 15 types of foods. Ten are considered "brain-healthy:" green leafy vegetables, other vegetables, nuts, berries, beans, whole grains, seafood, poultry, olive oil, and wine. Five are considered unhealthy: red meat, butter and stick margarine, cheese, pastries, sweets and fried/fast foods.
Researchers examined information from 5,907 older adults who participated in the Health and Retirement Study. The participants filled out questionnaires about their eating habits. Researchers then measured the participants' cognitive abilities -- mostly on their memory and attention skills.
The researchers compared the diets of participants to their performance on the cognitive tests. They found that older people who ate Mediterranean and MIND-style diets scored significantly better on the cognitive function tests than those who ate less healthy diets. In fact, older people who ate a Mediterranean-style diet had 35% lower risk of scoring poorly on cognitive tests. Even those who ate a moderate Mediterranean-style diet had 15% lower risk of doing poorly on cognitive tests. The researchers noted similar results for people who ate MIND-style diets.
This study suggests that eating Mediterranean and MIND-style diets is linked to better overall cognitive function in older adults, said the researchers. What's more, older adults who followed these healthy diets had lower risks for having cognitive impairment in later life, noted the researchers.

A changing society: 100 is the new 80
Centenarians healthier than previously thought during last years of life

When it comes to aging successfully and remaining in good health, are centenarians the perfect role models? Researchers have been studying illness trajectories in centenarians during the final years of their lives. According to their findings, people who died aged 100 or older suffered fewer diseases than those who died aged 90 to 99, or 80 to 89.
Forty years ago, life expectancy was such that, in the industrialized world, only (approximately) one in 10,000 people were expected to reach the age of 100 or more. Today's estimates suggest that half of all children born in the developed world during this century will live to at least 100. Therefore, the question that poses itself is whether extreme old age is necessarily associated with increased morbidity. There is evidence to suggest that centenarians develop fewer diseases than younger cohorts of extreme old people. In discussions surrounding the issues associated with aging populations, this is referred to as the 'compression of morbidity' hypothesis -- a term which describes the phenomenon of the onset of disability and age-related diseases being increasingly being well into old age, resulting in a shortening (or compression) of this phase. "Our aim was to gain a better understanding of multimorbidity, i.e. the number and severity of chronic diseases affecting centenarians towards the end of their lives," explains Dr. Paul Gellert of Charité's Institute of Medical Sociology and Rehabilitation Science.
Using diagnoses and health care utilization data routinely collected by the German statutory health insurance company Knappschaft, the researchers studied relevant events during the final six years of life of approximately 1,400 of the oldest old. For the purposes of analysis, this cohort was then divided into three groups. Data on persons who had died aged 100 or older were compared with random samples of persons who had died in their eighties or nineties. The analysis, which included data on very old persons living in their own homes as well as data on those living in residential care, focused on comorbid conditions classified by the Elixhauser Comorbidity Index as being usually associated with in-hospital mortality. "According to the data, centenarians suffered from an average of 3.3 such conditions during the three months prior to their deaths, compared with an average of 4.6 conditions for those who had died in their eighties," says Dr. Gellert, summarizing the findings. "Our results also show that the increase in conditions seen during the last few years of life was lower in centenarians than in those who had died between the ages of 90 and 99, or 80 and 89."
If one includes disorders commonly associated with extreme old age, such as different types of dementia and musculoskeletal disorders, approximately half of all centenarians recorded a total of five or more comorbid conditions. The same number of comorbid conditions was found in 60 percent of persons who had died in their nineties and 66 percent of persons who had died in their eighties. While different types of dementia and heart failure were found to be more common among centenarians than among the younger cohorts, high blood pressure, cardiac arrhythmia, renal failure, and chronic diseases were less common in those who had died after reaching 100 years of age. The incidence of musculoskeletal disorders was found to be similar in all three age groups. While there appears to be a clear link between extreme old age and the number of diseases recorded, the extent to which this is the case requires careful analysis.

Day-to-day experiences affect awareness of aging, mood

A study of older adults finds an individual's awareness of aging is not as static as previously thought, and that day-to-day experiences and one's attitude toward aging can affect an individual's awareness of age-related change -- and how that awareness affects one's mood.
"People tend to have an overall attitude toward aging, good or bad, but we wanted to know whether their awareness of their own aging -- or AARC -- fluctuated over time in response to their everyday experiences," says Shevaun Neupert, an associate professor of psychology at North Carolina State University and lead author of a paper on the study.
For the study, researchers enrolled 116 participants between the ages of 60 and 90. Each participant took a survey to establish baseline attitudes toward aging. For the following eight days, participants kept a log of daily stressors (such as having an argument), completed a daily evaluation of age-related experiences (such as "I am becoming wiser" or "I am more slow in my thinking"), and reported on their affect, or mood.
"We found that people's AARC, as reflected in their daily evaluations, varied significantly from day to day," says Jennifer Bellingtier, a recent Ph.D. graduate from NC State and co-author of the paper. "We also found that people whose baseline attitudes toward aging were positive also tended to report more positive affect, or better moods."
"People with positive attitudes toward aging were also less likely to report 'losses,' or negative experiences, in their daily aging evaluations," Neupert says.
"However, when people with positive attitudes did report losses, it had a much more significant impact on their affect that day," Neupert says. "In other words, negative aging experiences had a bigger adverse impact on mood for people who normally had a positive attitude about aging."
The study expands on previous work that found having a positive attitude about aging makes older adults more resilient when faced with stressful situations.

Natural plant compound may reduce mental effects of aging, more evidence shows

The benefits of antioxidant fisetin have been demonstrated in mouse model of premature aging, Alzheimer's disease.
Salk scientists have found further evidence that a natural compound in strawberries reduces cognitive deficits and inflammation associated with aging in mice. The work, which appeared in the Journals of Gerontology Series A in June 2017, builds on the team's previous research into the antioxidant fisetin, finding it could help treat age-related mental decline and conditions like Alzheimer's or stroke.
"Companies have put fisetin into various health products but there hasn't been enough serious testing of the compound," says Pamela Maher, a senior staff scientist in Salk's Cellular Neurobiology Laboratory and senior author of the paper. "Based on our ongoing work, we think fisetin might be helpful as a preventative for many age-associated neurodegenerative diseases, not just Alzheimer's, and we'd like to encourage more rigorous study of it."
Maher, who works in the lab of David Schubert, the head of Salk's Cellular Neurobiology Lab, has been studying fisetin for over a decade. Previous research by the lab found that fisetin reduced memory loss related to Alzheimer's in mice genetically modified to develop the disease. But that study focused on genetic (familial) AD, which accounts for only 1 to 3 percent of cases. By far the bigger risk factor for developing what is termed sporadic AD, as well as other neurodegenerative disorders, is simply age. For the current inquiry, Maher turned to a strain of laboratory mice that age prematurely to better study sporadic AD. By 10 months of age, these mice typically show signs of physical and cognitive decline not seen in normal mice until two years of age.
The Salk team fed the 3-month-old prematurely aging mice a daily dose of fisetin with their food for 7 months. Another group of the prematurely aging mice was fed the same food without fisetin. During the study period, mice took various activity and memory tests. The team also examined levels of specific proteins in the mice related to brain function, responses to stress and inflammation.
"At 10 months, the differences between these two groups were striking," says Maher. Mice not treated with fisetin had difficulties with all the cognitive tests as well as elevated markers of stress and inflammation. Brain cells called astrocytes and microglia, which are normally anti-inflammatory, were now driving rampant inflammation. Mice treated with fisetin, on the other hand, were not noticeably different in behavior, cognitive ability or inflammatory markers at 10 months than a group of untreated 3-month-old mice with the same condition. Additionally, the team found no evidence of acute toxicity in the fisetin-treated mice, even at high doses of the compound.
"Mice are not people, of course," says Maher, "But there are enough similarities that we think fisetin warrants a closer look, not only for potentially treating sporadic AD but also for reducing some of the cognitive effects associated with aging, generally."
Next, Maher hopes to partner with another group or company in order to conduct clinical trials of fisetin with human subjects.

Having a purpose in life means you are more likely to sleep better at night with less sleep apnea and restless leg syndrome, reports a new study. Cultivating a purpose in life could be drug-free strategy to improve sleep, scientists said. The study participants were older adults -- who tend to have more insomnia and sleep disturbances -- but researchers said the findings are likely applicable to the broader public.
Having a good reason to get out of bed in the morning means you are more likely to sleep better at night with less sleep apnea and restless leg syndrome, reports a new Northwestern Medicine and Rush University Medical Center study based on older adults.
This is the first study to show having a purpose in life specifically results in fewer sleep disturbances and improved sleep quality and over a long period of time. Previous research showed having a purpose in life generally improves overall sleep when measured at a single point in time.
Although the participants in the study were older, researchers said the findings are likely applicable to the broader public.
"Helping people cultivate a purpose in life could be an effective drug-free strategy to improve sleep quality, particularly for a population that is facing more insomnia," said senior author Jason Ong, an associate professor of neurology at Northwestern University Feinberg School of Medicine. "Purpose in life is something that can be cultivated and enhanced through mindfulness therapies."
The paper will be published in the journal Sleep Science and Practice.
Individuals have more sleep disturbances and insomnia as they get older. Clinicians prefer to use non-drug interventions to improve patients' sleep, a practice now recommended by the American College of Physicians as a first line treatment for insomnia, Ong said.
The next step in the research should be to study the use of mindfulness-based therapies to target purpose in life and resulting sleep quality, said Arlener Turner, the study's first author and a former postdoctoral fellow in neurology at Feinberg.
The 823 participants -- non-demented individuals 60 to 100 years old with an average age of 79 -- were from two cohorts at Rush University Medical Center. More than half were African American and 77 percent were female.
People who felt their lives had meaning were 63 percent less likely to have sleep apnea and 52 percent less likely to have restless leg syndrome. They also had moderately better sleep quality, a global measure of sleep disturbance.
For the study, participants answered a 10-question survey on purpose in life and a 32-question survey on sleep. For the purpose in life survey, they were asked to rate their response to such statements as, "I feel good when I think of what I've done in the past and what I hope to do in the future."
The next step in the research should be to study the use of mindfulness-based therapies to target purpose in life and resulting sleep quality, Turner said.
Poor sleep quality is related to having trouble falling asleep, staying asleep and feeling sleepy during the day. Sleep apnea is a common disorder that increases with age in which a person has shallow breathing or pauses in breathing during sleep several times per hour. This disruption often makes a person feel unrefreshed upon waking up and excessively sleepy during the day.
Restless leg syndrome causes uncomfortable sensations in the legs and an irresistible urge to move them. Symptoms commonly occur in the late afternoon or evening hours and are often most severe at night when a person is resting, such as sitting or lying in bed.

Well-being in later life is largely dependent on psychosocial factors. Physical impairments tend to play a secondary role, as scientists have discovered.
"Aging itself is not inevitably associated with a decline in mood and quality of life," says Prof. Karl-Heinz Ladwig, summarizing the results. "It is rather the case that psychosocial factors such as depression or anxiety impair subjective well-being, the Head of the Mental Health Research Group at the Institute of Epidemiology II, Helmholtz Zentrum München and Professor of Psychosomatic Medicine at the TUM University Hospital explains. "And in the case of women, living alone also plays an important role."
"To date the impact of emotional stress has barely been investigated"
For the current study, Prof. Ladwig and his team relied on data derived from about 3,600 participats with an average age of 73 who had taken part in the population-based KORA-Age Study. "What made the study particularly interesting was the fact that the impact of stress on emotional well-being has barely been investigated in a broader, non-clinical context," explains PD Dr. Karoline Lukaschek, epidemiologist in the Mental Health Research Group and lead author of the paper. "Our study therefore explicitly included anxiety, depression and sleep disorders."
Generally high levels of well-being but...
To ascertain levels of subjective well-being, the scientists used a questionnaire devised by the World Health Organization (the WHO-5 Well-Being Index) with a score range of 0 to 100. For the purpose of analysis, they divided the respondents' results into two categories: 'high' (score > 50) and 'low' (score ? 50). The subsequent evaluation revealed a high level of subjective well-being in the majority (79 percent) of the respondents. The average values were also above the threshold set by the WHO. In the 'low' group, however, there was a conspicuously high number of women: about 24 percent compared to 18 percent for men.
Depression and anxiety disorders are the biggest risk
Trying to uncover the most important causes for subjective well-being, the scientists mainly identified psychosocial factors: above all, depression and anxiety disorders had the strongest effect on well-being. Low income and sleep disorders also had a negative effect. However, poor physical health (for example, low physical activity or so-called multimorbidity) seemed to have little impact on perceived life satisfaction. Among women, living alone also significantly increased the probability of a low sense of well-being.
"The findings of the current study clearly demonstrate that appropriate services and interventions can play a major role for older people, especially for older women living on their own," Prof. Ladwig says, categorizing the results. "And this is all the more important, given that we know that high levels of subjective well-being are linked to a lower mortality risk."

Trouble Sleeping? It May Affect Your Memory Later On

The amount and quality of sleep you get at night may affect your memory later in life, according to research that was recently released and will be presented at the American Academy of Neurology's 64th Annual Meeting in New Orleans April 21 to April 28, 2012.
"Disrupted sleep appears to be associated with the build-up of amyloid plaques, a hallmark marker of Alzheimer's disease, in the brains of people without memory problems," said study author Yo-El Ju, MD, with Washington University School of Medicine in St. Louis and a member of the American Academy of Neurology. "Further research is needed to determine why this is happening and whether sleep changes may predict cognitive decline."
Researchers tested the sleep patterns of 100 people between the ages of 45 and 80 who were free of dementia. Half of the group had a family history of Alzheimer's disease. A device was placed on the participants for two weeks to measure sleep. Sleep diaries and questionnaires were also analyzed by researchers.
After the study, it was discovered that 25 percent of the participants had evidence of amyloid plaques, which can appear years before the symptoms of Alzheimer's disease begin. The average time a person spent in bed during the study was about eight hours, but the average sleep time was 6.5 hours due to short awakenings in the night.
The study found that people who woke up more than five times per hour were more likely to have amyloid plaque build-up compared to people who didn't wake up as much. The study also found those people who slept "less efficiently" were more likely to have the markers of early stage Alzheimer's disease than those who slept more efficiently. In other words, those who spent less than 85 percent of their time in bed actually sleeping were more likely to have the markers than those who spent more than 85 percent of their time in bed actually sleeping.
"The association between disrupted sleep and amyloid plaques is intriguing, but the information from this study can't determine a cause-effect relationship or the direction of this relationship. We need longer-term studies, following individuals' sleep over years, to determine whether disrupted sleep leads to amyloid plaques, or whether brain changes in early Alzheimer's disease lead to changes in sleep," Ju said.
"Our study lays the groundwork for investigating whether manipulating sleep is a possible strategy in the prevention or slowing of Alzheimer disease."

The results may lead the way to new rehabilitative therapies for speech disorders.
When a stroke damages speech areas in the brain's left hemisphere, sufferers often have severe difficulties speaking -- a condition known as non-fluent aphasia. Sometimes the inability to speak spontaneously is permanent. However, there are frequent cases of aphasics who are able to sing song lyrics and formulaic phrases relatively fluently. Until now, this astonishing observation has been explained by the fact that the right brain hemisphere, which supports important functions of singing, remains intact. Singing was thought to stimulate areas in the right hemisphere, which would then assume the function for damaged left speech areas. A treatment method known as Melodic Intonation Therapy is based on this idea.
Recent research has shown that changes indeed occur in the right brain hemisphere of patients after singing formulaic phrases like 'How are you?' over a period of months. "But this alone is not sufficient evidence that singing is an effective treatment for aphasics," says Benjamin Stahl, researcher at the Max Planck Institute for Human Cognitive and Brain Sciences in Leipzig. "The formulaic phrases could just as easily be the cause, as similar areas of the right brain hemisphere are activated when such texts are produced." Moreover, one should not jump to conclusions, Stahl says: "Changes in the right brain hemisphere are not necessarily the cause of improvement in a patient's articulation."

Zinc regulates communication between brain cells

Zinc has been found to play a critical role in regulating communication between cells in the brain, possibly governing the formation of memories and controlling the occurrence of epileptic seizures.
A collaborative project between Duke University Medical Center researchers and chemists at the Massachusetts Institute of Technology has been able to watch zinc in action as it regulates communication between neurons in the hippocampus, where learning and memory processes occur -- and where disrupted communication may contribute to epilepsy.
"We discovered that zinc is essential to control the efficiency of communication between two critical populations of nerve cells in the hippocampus," said James McNamara, M.D., senior author and chair of the Department of Neurobiology at Duke. "This addresses a longstanding controversy in the field."
"Carefully controlling zinc's regulation of communication between these nerve cells is critical to both formation of memories and perhaps to occurrence of epileptic seizures," McNamara said.

Aerobic exercise may reduce the risk of dementia

Any exercise that gets the heart pumping may reduce the risk of dementia and slow the condition's progression once it starts, a new study finds. Researchers examined the role of aerobic exercise in preserving cognitive abilities and concluded that it should not be overlooked as an important therapy against dementia.
"We culled through all the scientific literature we could find on the subject of exercise and cognition, including animal studies and observational studies, reviewing over 1,600 papers, with 130 bearing directly on this issue. We attempted to put together a balanced view of the subject," says J. Eric Ahlskog, M.D., Ph.D., a neurologist at Mayo Clinic.
"We concluded that you can make a very compelling argument for exercise as a disease-modifying strategy to prevent dementia and mild cognitive impairment, and for favorably modifying these processes once they have developed."
The researchers note that brain imaging studies have consistently revealed objective evidence of favorable effects of exercise on human brain integrity. Also, they note, animal research has shown that exercise generates trophic factors that improve brain functioning, plus exercise facilitates brain connections (neuroplasticity).

Older adults who lead sedentary lifestyles and consume a lot of sodium in their diet may be putting themselves at risk for more than just heart disease. A new study has found evidence that high-salt diets coupled with low physical activity can be detrimental to cognitive health in older adults.
"The results of our study showed that a diet high in sodium, combined with little exercise, was especially detrimental to the cognitive performance of older adults," said Dr. Fiocco. "But the good news is that sedentary older adults showed no cognitive decline over the three years that we followed them if they had low sodium intake."
"These data are especially relevant as we know that munching on high-salt processed snacks when engaged in sedentary activities, such as watching TV or playing in front of the computer, is a frequent pastime for many adults," said Dr. Carol Greenwood, a senior author on the study and internationally-renowned scientist in the field of nutrition and cognitive function in late life.

Fish oil's impact on cognition and brain structure

Researchers have found positive associations between fish oil supplements and cognitive functioning as well as differences in brain structure between users and non-users of fish oil supplements. The findings suggest possible benefits of fish oil supplements on brain health and aging.
Daiello says, "In the imaging analyses for the entire study population, we found a significant positive association between fish oil supplement use and average brain volumes in two critical areas utilized in memory and thinking (cerebral cortex and hippocampus), as well as smaller brain ventricular volumes compared to non-users at any given time in the study. In other words, fish oil use was associated with less brain shrinkage in patients taking these supplements during the ADNI study compared to those who didn't report using them."
Daiello continues, "These observations should motivate further study of the possible effects of long-term fish oil supplementation on important markers of cognitive decline and the potential influence of genetics on these outcomes."
