Lack of sleep lessens cognitive benefits of physical activity

A new study looked at cognitive function over 10 years in 8,958 people aged 50 and over in England. The research team investigated how different combinations of sleep and physical activity habits might affect people's cognitive function over time.

Regular physical activity may protect against cognitive decline as we get older, but this protective effect may be diminished for people who are not getting enough sleep, according to a new study by UCL researchers.

The study, published in The Lancet Healthy Longevity, looked at cognitive function over 10 years in 8,958 people aged 50 and over in England. The research team investigated how different combinations of sleep and physical activity habits might affect people's cognitive function over time.

They found that people who were more physically active but had short sleeps -- less than six hours on average -- had faster cognitive decline overall, meaning that after 10 years their cognitive function was equivalent to peers who did less physical activity.

Lead author Dr Mikaela Bloomberg (UCL Institute of Epidemiology & Health Care) said: "Our study suggests that getting sufficient sleep may be required for us to get the full cognitive benefits of physical activity. It shows how important it is to consider sleep and physical activity together when thinking about cognitive health.

"Previous studies examining how sleep and physical activity might combine to affect cognitive function have primarily been cross-sectional -- only focusing on a snapshot in time -- and we were surprised that regular physical activity may not always be sufficient to counter the long-term effects of lack of sleep on cognitive health."

The study found, in line with previous research, that sleeping between six and eight hours per night and higher levels of physical activity were linked to better cognitive function.

Those who were more physically active also had better cognitive function regardless of how long they slept at the start of the study. This changed over the 10-year period, with more physically active short sleepers (less than six hours) experiencing more rapid cognitive decline.

This rapid decline was true for those in their 50s and 60s in this group, but for older participants (aged 70 and over) the cognitive benefits of exercise appeared to be maintained, despite short sleep.

Co-author Professor Andrew Steptoe (UCL Institute of Epidemiology & Health Care) said: "It is important to identify the factors that can protect cognitive function in middle and later life as they can serve to prolong our cognitively healthy years and, for some people, delay a dementia diagnosis.

"The World Health Organisation already identifies physical activity as a way to maintain cognitive function, but interventions should also consider sleep habits to maximise long-term benefits for cognitive health."

For the study, the researchers used data from the English Longitudinal Study of Ageing (ELSA), a nationally representative cohort study of the English population. Participants were asked how long they slept on an average weeknight and were split into three sleep groups: short (less than six hours), optimal (six to eight hours) and long (greater than eight hours).

They were also given a score based on the frequency and intensity of self-reported physical activity and divided into two groups: more physically active (the top third of scorers) and less physically active (the other two thirds). Cognitive function was assessed on the basis of an episodic memory test (asking participants to recall a 10-word list, both immediately and after a delay) and a verbal fluency test (asking participants to name as many animals as they can in a minute).

The researchers adjusted for a number of confounding factors, such as participants having done the same cognitive test before and therefore being likely to perform better. They also excluded people with self-reported dementia diagnoses and those whose test scores indicated some cognitive impairment, so that behaviour changes linked to preclinical Alzheimer's disease (such as sleep disturbance) did not inadvertently affect the results.

In terms of study limitations, the researchers relied on participants self-reporting their sleep duration and physical activity. The next steps, the researchers said, may be to repeat the results in more diverse study populations, examine more cognitive domains and more domains of sleep quality, and use objective measures such as a wearable physical activity tracker.

The research was funded by the UK's Economic and Social Research Council.

Study of 600 UK teenagers suggests that having stronger self-awareness and sense of purpose may raise GCSE Maths scores 'by a couple of grades'.

Encouraging adolescents to feel capable and purposeful -- rather than just happy -- could improve their academic results as well as their mental health, according to new research which recommends changing how wellbeing is supported in schools.

The University of Cambridge study, involving over 600 teenagers from seven English schools, examined two separate aspects of their wellbeing: life satisfaction and 'eudaimonia'. While life satisfaction roughly equates to how happy a person is, eudaimonia refers to how well that person feels they are functioning. It incorporates feelings of competence, motivation and self-esteem.

Researchers found that students with high levels of eudaimonia consistently outperformed their peers in GCSE-level assessments, especially Maths. On average, those achieving top Maths grades had eudaimonic wellbeing levels 1.5 times higher than those with the lowest grades.

No such link was found between academic performance and life satisfaction. Despite this, child wellbeing policy in England tends to focus on life satisfaction. The Government has, for example, recently added 'happiness' to national curricula as part of its Relationships, Sex and Health Education (RSHE) guidance, emphasising teaching adolescents how to feel happy and resilient while managing negative emotions.

Previous research has pointed to the importance of fostering adolescents' eudaimonic wellbeing by nurturing their personal values, goals and sense of self-worth. The new study appears to strengthen that case by demonstrating a positive link between eudaimonia and academic performance.

Its lead author, Dr Tania Clarke, is a psychologist of education who now works for the Youth Endowment Fund, but undertook the study for her doctoral research at the Faculty of Education, University of Cambridge. The findings are published in School Psychology Review.

"Wellbeing education often focuses on teaching students about being happy and not being sad." Clarke said. "That is over-simplistic and overlooks other vital qualities of wellbeing that are particularly salient during the formative period of adolescence."

"Adolescents also need to develop self-awareness, confidence, and ideally a sense of meaning and purpose. Judging by our findings, an adolescent who is currently getting a 3 or 4 on their Maths GCSE could be helped to rise a couple of grades if schools emphasised these qualities for all students, rather than just promoting positivity and minimising negative emotions."

The study involved 607 adolescents, aged 14-15. Participants completed an established psychological assessment called 'How I feel about myself and school', which measures both life satisfaction and eudaimonia, as well as feelings of interpersonal relatedness and negativity.

These measures were compared with their scores in mock English and Maths GCSEs. The research also assessed whether the students exhibited a 'growth mindset': a belief in their personal capacity for improvement. Many educators consider this essential for enhancing academic performance.

The students' overall wellbeing -- their eudaimonia and life satisfaction combined -- clearly correlated positively with their exam results. Those attaining top Maths grades (Grades 8 or 9) had, on average, a wellbeing score of 32 out of a possible 50. This was nine points higher than those with a Grade 1, and three to four points higher than the average for all 607 students.

When they analysed the separate dimensions of wellbeing, however, the researchers found a positive relationship between eudaimonia and higher attainment, but no correlation with life satisfaction. In Maths, the average eudaimonic wellbeing score of Grade 9 students was 17.3 from a possible 25, while that of Grade 1 students was just 10.9. These results held true even when accounting for potentially confounding factors, such as school attended, gender, socio-economic status, or special educational needs.

The study also found that a growth mindset did not predict good academic results, although students with high eudaimonic wellbeing did tend to exhibit such a mindset. Other research has similarly struggled to draw a clear link between growth mindset and academic progress, but does link it more generally to positive mental health. This implies that eudaimonia, as well as supporting better attainment, may also underpin important aspects of self-belief, leading to broader mental health benefits.

Clarke's wider research suggests that various constraints currently limit schools' capacity to promote eudaimonic wellbeing. In an earlier Review of Education article she published the results of in-depth interviews with some of the same students, which highlighted concerns about a 'performativity culture' stemming from a heavy emphasis on high-stakes testing. These interviews indicated that many students associate 'doing well' with getting good grades, rather than with their own strengths, values and goals.

Students said they often felt worthless, inadequate or "dumb" if they failed to get high marks in tests. "You let your scores define you," one student told Clarke. "Then you feel really low about… your worth and everything. You think it's literally the end of the world." Ironically, the new findings suggest that by limiting teachers' capacity to support students' personal growth, the heavy emphasis on exam results and testing may be undermining academic progress, at least in some cases.

Clarke suggested that eudaimonic therapy, which increasingly features in professional mental health psychology for adolescents, could be incorporated more into wellbeing education. In particular, her study underscores the need to help students understand their academic work and progress in the context of their personal motivations and goals.

"There is a link between better wellbeing and a more nuanced understanding of academic success," Clarke said. "Because schools are under heavy pressure to deliver academic results, at the moment students seem to be measuring themselves against the exam system, rather than in terms of who they want to be or what they want to achieve."

Dr Ros McLellan, from the Faculty of Education, University of Cambridge, who co-authored the study, said: "Wellbeing education needs to move beyond notions of 'boosting' happiness towards deeper engagement, helping adolescents to realise their unique talents and aspirations, and a sense of what happiness means for them, personally. This would not just improve wellbeing: it is also likely to mean better exam results, and perhaps fewer issues for students later on."

Fluctuating levels of cholesterol and triglycerides linked to increased risk of dementia

Older people who have fluctuating levels of cholesterol and triglycerides may have a higher risk of Alzheimer's disease and related dementias compared to people who have steady levels, according to new research published in the July 5, 2023, online issue of Neurology®, the medical journal of the American Academy of Neurology. While the study found a link, it does not prove that fluctuating levels of cholesterol and triglycerides cause dementia.

"Prevention strategies for Alzheimer's and related dementias are urgently needed," said study author Suzette J. Bielinski, PhD, of the Mayo Clinic in Rochester, Minnesota. "Routine screenings for cholesterol and triglyceride levels are commonly done as part of standard medical care. Fluctuations in these results over time could potentially help us identify who is at greater risk for dementia, help us understand mechanisms for the development of dementia and ultimately determine whether leveling out these fluctuations could play a role in reducing dementia risk."

Researchers used health care data to identify 11,571 people age 60 or older who did not have a prior diagnosis of Alzheimer's disease or dementia. Researchers looked at participants' measurements of total cholesterol, triglycerides, low-density lipoprotein cholesterol (LDL) and high-density lipoprotein cholesterol (HDL) on at least three different days in the five years before the start of the study.

Then researchers divided participants into five equal groups based on how much the measurements fluctuated. The lowest group had the least variation over time and the highest group had the most variation.

Participants were followed for an average of 13 years. During that time, 2,473 people developed Alzheimer's disease or another form of dementia.

After adjusting for variables that could affect risk of dementia including sex, race, education and lipid-lowering treatments, researchers found for total cholesterol, participants in the highest group had a 19% increased risk of dementia compared to those in the lowest group. Of the 2,311 people in the highest group, 515 developed dementia compared to 483 of the 2,311 people in the lowest group. For triglycerides, those in highest group had a 23% increased risk.

Researchers did not find a link between variations in LDL and HDL and an increased risk of dementia.

"It remains unclear why and how fluctuating levels of cholesterol and triglycerides are related to the risk of Alzheimer's disease," said Bielinski. "Further studies looking at the changes over time for this relationship are needed in order to confirm our results and potentially consider preventative strategies."

A limitation of the study was researchers looked at Alzheimer's disease and related dementias as a whole and did not differentiate between the types of dementia.

The study was supported by the National Heart, Lung and Blood Institute.

Taking good care of your teeth may be good for your brain

Poor dental health linked to decline in brain volume

Taking good care of your teeth may be linked to better brain health, according to a study published in the July 5, 2023, online issue of Neurology®, the medical journal of the American Academy of Neurology. The study found that gum disease and tooth loss were linked to brain shrinkage in the hippocampus, which plays a role in memory and Alzheimer's disease. The study does not prove that gum disease or tooth loss causes Alzheimer's disease; it only shows an association.

"Tooth loss and gum disease, which is inflammation of the tissue around the teeth that can cause shrinkage of the gums and loosening of the teeth, are very common, so evaluating a potential link with dementia is incredibly important," said study author Satoshi Yamaguchi, PhD, DDS, of Tohoku University in Sendai, Japan. "Our study found that these conditions may play a role in the health of the brain area that controls thinking and memory, giving people another reason to take better care of their teeth."

The study involved 172 people with an average age of 67 who did not have memory problems at the beginning of the study.

Participants had dental exams and took memory tests at the beginning of the study. They also had brain scans to measure volume of the hippocampus at the beginning of the study and again four years later.

For each participant, researchers counted the number of teeth and checked for gum disease by looking at periodontal probing depth, a measurement of the gum tissue. Healthy readings are from one to three millimeters.

Mild gum disease involves probing depths of three or four millimeters in several areas, while severe gum disease involves probing depths of five or six millimeters in several areas as well as more bone loss and can cause teeth to become loose and eventually fall out.

Researchers found that the number of teeth and amount of gum disease was linked to changes in the left hippocampus of the brain.

For people with mild gum disease having fewer teeth was associated with a faster rate of brain shrinkage in the left hippocampus.

However, for people with severe gum disease having more teeth was associated with a faster rate of brain shrinkage in the same area of the brain.

After adjusting for age, researchers found that for people with mild gum disease, the increase in the rate of brain shrinkage due to one less tooth was equivalent to nearly one year of brain aging. Conversely, for people with severe gum disease the increase in brain shrinkage due to one more tooth was equivalent to 1.3 years of brain aging.

"These results highlight the importance of preserving the health of the teeth and not just retaining the teeth," Yamaguchi said. "The findings suggest that retaining teeth with severe gum disease is associated with brain atrophy. Controlling the progression of gum disease through regular dental visits is crucial, and teeth with severe gum disease may need to be extracted and replaced with appropriate prosthetic devices."

Yamaguchi said future studies are needed with larger groups of people. Another limitation of the study is that it was conducted in one region of Japan, so the results may not be generalizable to other locations.

Depression after traumatic brain injury could represent a new, distinct disease

A new study led by Shan Siddiqi, MD, from Brigham and Women's Hospital, a founding member of the Mass General Brigham healthcare system, suggests that depression after traumatic brain injury (TBI) could be a clinically distinct disorder rather than traditional major depressive disorder, with implications for patient treatment. The findings are published in Science Translational Medicine.

"Our findings help explain how the physical trauma to specific brain circuits can lead to development of depression. If we're right, it means that we should be treating depression after TBI like a distinct disease," said corresponding author Shan Siddiqi, MD, of the Brigham's Department of Psychiatry and Center for Brain Circuit Therapeutics. "Many clinicians have suspected that this is a clinically distinct disorder with a unique pattern of symptoms and unique treatment response, including poor response to conventional antidepressants -- but until now, we didn't have clear physiological evidence to prove this."

Siddiqi collaborated with researchers from Washington University in St. Louis, Duke University School of Medicine, the University of Padua, and the Uniformed Services University of the Health Sciences on the study. The work started as a side project seven years ago when Siddiqi was motivated by a patient he shared with David Brody, MD, PhD, a co-author on the study and a neurologist at Uniformed Services University. The two started a small clinical trial that used personalized brain mapping to target brain stimulation as a treatment for TBI patients with depression. In the process, they noticed a specific pattern of abnormalities in these patients' brain maps.

The current study included 273 adults with TBI, usually from sports injuries, military injuries, or car accidents. People in this group were compared to other groups who did not have a TBI or depression, people with depression without TBI, and people with posttraumatic stress disorder. Study participants went through a resting-state functional connectivity MRI, a brain scan that looks at how oxygen is moving in the brain. These scans gave information about oxygenation in up to 200,000 points in the brain at about 1,000 different points in time, leading to about 200 million data points in each person. Based on this information, a machine learning algorithm was used to generate an individualized map of each person's brain.

The location of the brain circuit involved in depression was the same among people with TBI as people without TBI, but the nature of the abnormalities was different. Connectivity in this circuit was decreased in depression without TBI and was increased in TBI-associated depression. This implies that TBI-associated depression may be a different disease process, leading the study authors to propose a new name: "TBI affective syndrome."

"I've always suspected it isn't the same as regular major depressive disorder or other mental health conditions that are not related to traumatic brain injury," said Brody. "There's still a lot we don't understand, but we're starting to make progress."

One limitation of the trial is that with so much data, the researchers were not able to do detailed assessments of each patient beyond brain mapping. As a future step, investigators would like to assess participants' behavior in a more sophisticated way and potentially define different kinds of TBI-associated neuropsychiatric syndromes.

Siddiqi and Brody are also using this approach to develop personalized treatments. Originally, they set out to design a new treatment in which they used this brain mapping technology to target a specific brain region for people with TBI and depression, using transcranial magnetic stimulation (TMS). They enrolled 15 people in the pilot and saw success with the treatment. Since then, they have received funding to replicate the study in a multicenter military trial.

"We hope our discovery guides a precision medicineapproach to managing depression and mild TBI, and perhaps even intervene in neuro-vulnerable trauma survivors before the onset of chronic symptoms," said Rajendra Morey, MD, a professor of psychiatry at Duke University School of Medicine, and co-author on the study.

Memories of childhood abuse and neglect has greater impact on mental health than the experience itself

New research from the Institute of Psychiatry, Psychology & Neuroscience (IoPPN) at King's College London and City University New York, published today (Wednesday 5 July) in JAMA Psychiatry, has found that the way childhood abuse and/or neglect is remembered and processed has a greater impact on later mental health than the experience itself. The authors suggest that, even in the absence of documented evidence, clinicians can use patients' self-reported experiences of abuse and neglect to identify those at risk of developing mental health difficulties and provide early interventions.

Researchers conducted a large longitudinal study following 1,196 participants to age 40 years to investigate how experiences of childhood abuse and/or neglect (maltreatment) impact the development of emotional disorders in adulthood.

The study found that young adults who retrospectively self-reported experiences of childhood maltreatment before age 12 had a greater number of depressive or anxiety episodes over the subsequent decade than those who did not remember maltreatment, even if they had an official court record.

In contrast, participants who had an official record of childhood maltreatment, but no retrospective recall of the experience, had a similar number of emotional disorder episodes in adulthood as those with no experience of maltreatment.

Andrea Danese, Professor of Child & Adolescent Psychiatry at King's IoPPN and joint author of the study, said: "Our study reveals that how a person perceives and remembers experiences of childhood abuse or neglect has greater implications on future emotional disorders than the experience itself. The findings show that, even in the absence of documented evidence of childhood maltreatment, clinicians can use information provided by their clients to identify those at greater risk for subsequent mental health difficulties. The findings also suggest that early interventions that help cope with memories of abuse and/or neglect may prevent emotional problems later on."

Participants were interviewed about their self-reported retrospective experiences of childhood maltreatment and their current and past mental health. They were then re-interviewed to measure the course of depression and anxiety symptoms.

Further analyses revealed that the association between self-reported experiences of childhood maltreatment and a greater number of subsequent anxiety and depression episodes was partly explained by participants' current and past mental health, which was reported during their first interview. The authors explain that this could be because emotional disorders can negatively bias memories, making participants' more likely to recall negative events.

Professor Danese said: "A better understanding of how memories of child maltreatment are maintained and exacerbated over time, and of how the memories affect daily functioning, could provide new insights to develop effective interventions."

Treating childhood ADHD with stimulant meds not associated with increased substance use later in life

Children taking a prescription stimulant to manage symptoms of attention deficit hyperactivity disorder (ADHD) do not have more substance use or substance use disorder (SUD) as adolescents or young adults, according to a new study by researchers at the University of Pittsburgh School of Medicine.

Published today in JAMA Psychiatry, the study may provide some reassurance to parents and clinicians who may be hesitant to prescribe ADHD stimulant medications for fear that they may lay the groundwork for future substance use.

"Stimulants are the first-line treatment recommended for most individuals with ADHD -- the drug class is an evidence-based treatment with few side effects," said Brooke Molina, Ph.D., professor of psychiatry, psychology and pediatrics at Pitt. "Because stimulant medications are classified by the Drug Enforcement Administration as schedule two substances with the potential for misuse, many people fear that harmful substance use could result."

ADHD is a neurobehavioral condition marked by chronic patterns of inattention, hyperactivity or impulsivity, or a combination of the three, that affects a person's day-to-day functioning. Although ADHD can be managed with medication and other therapeutic approaches, it is a chronic condition that must be monitored throughout an individual's life.

Molina and her colleagues assessed patients with ADHD over a 16-year period from childhood through adolescence to early adulthood to see if there was any association between stimulant treatment and subsequent substance use. The study conducted at Pitt is among the first to address the relationship between childhood use of prescription stimulants and later SUD by accounting for dozens of demographic, clinical and psychosocial factors that may predispose an individual to treatment and substance use.

"Our study not only accounted for age, but also used a statistical method that adjusted over time for the many characteristics that may distinguish treated from non-treated individuals," said study co-author Traci Kennedy, Ph.D., assistant professor of psychiatry at Pitt. "Considering these factors allowed us to more accurately test the relationship between stimulants and substance use."

While other studies have sought to uncover and define a possible connection between prescription stimulant use for ADHD and SUD, the association between the two has remained controversial. The results of some studies suggested a protective effect of prescription stimulant use on the risk of having SUD later in life, while others failed to find an association.

When factoring in age and other time-varying characteristics, such as household income, behavior problems and parental support, Pitt researchers found no evidence that prescription stimulant treatment in childhood provided protection against developing a SUD for adolescents or young adults with ADHD. However, researchers likewise did not find an association between stimulant use during childhood and increased substance misuse in the future

While some study participants self-reported an increase over time in heavy drinking, marijuana use, daily cigarette smoking and using other substances, an association with age was also found for stimulant treatment, with older participants being less likely to continue taking medication. When these trends were paired with rigorous statistical analysis, results provided no evidence that prolonged stimulant use is associated with reduced or increased risk for SUD.

"We hope the results of this study will help educate providers and patients," Molina said. "By understanding that stimulant medication initially prescribed in childhood is not linked to harmful levels of substance use, I anticipate that parents' and patients' fears will be alleviated."

Pitt researchers plan to study individuals who were first diagnosed with ADHD and treated with stimulants in adulthood. The study aims to learn if there are differences in the characteristics and outcomes of these adults compared to people who were diagnosed and first treated with stimulants in childhood.

Older adults who remain more active have a better quality of life

A reduction in the amount of time spent physically active when adults are over sixty years old is linked to lower quality of life, a Cambridge study of almost 1,500 adults has shown.

The same was also true for increases in the amount of sedentary time, such as watching TV or reading. The researchers say this highlights the need to encourage older adults to remain active.

Physical activity -- particularly when it is moderate-intensity and raises your heart rate -- is known to reduce the risk of a number of diseases, including heart disease, stroke, diabetes and cancer. The NHS recommends that adults do at least 150 minutes of moderate-intensity activity or 75 minutes of vigorous-intensity activity a week. Older adults are also recommended to break up prolonged periods of being sedentary with light activity when physically possible, or at least with standing, as this has distinct health benefits for older people.

A team led by researchers at the University of Cambridge examined activity levels among 1,433 participants aged 60 and above using accelerometers. The participants had been recruited to the EPIC (European Prospective Investigation into Cancer)-Norfolk study.

Alongside this, the team also looked at health-related quality of life, a measure of health and wellbeing that includes pain, ability to care for yourself and anxiety/mood. Participants were given a score between 0 (worst quality of life) and 1 (best) based on their responses to a questionnaire. Lower quality of life scores are linked with an increased risk of hospitalisation, worse outcomes following hospitalisation, and early death.

Participants were followed up an average of just under six years later to look at changes in their behaviour and quality of life. The results of the study are published in Health and Quality of Life Outcomes.

On average, six years after their first assessment, both men and women were doing around 24 minutes less moderate-to-vigorous physical activity per day. At the same time, the total sedentary time increased by an average of around 33 minutes a day for men and around 38 minutes a day for women.

Those individuals who did more moderate-to-vigorous physical activity and spent less time sedentary at their first assessment had a higher quality of life later on. An hour a day spent more active was associated with a 0.02 higher quality of life score.

For every minute a day less of moderate-to-vigorous physical activity measured six years after the first assessment, quality of life scores dropped by 0.03. This means that an individual who spent 15 minutes a day less engaged in such activity would have seen their score drop by 0.45.

Increases in sedentary behaviours were also associated with poorer quality of life -- a drop in the score of 0.012 for everyone minute a day increase in total sedentary time six years after the first measurement. This means that an individual who spent 15 minutes a day more sitting down would have seen their score drop by 0.18.

To put the results into a clinical context, a 0.1 point improvement in quality of life scores has previously been associated with a 6.9% reduction in early death and a 4.2% reduction in risk of hospitalisation.

Dr Dharani Yerrakalva from the Department of Public Health and Primary Care at the University of Cambridge said: "Keeping yourself active and limiting -- and where you can, breaking up -- the amount of time you spend sitting down is really important whatever stage of life you're at. This seems to be particularly important in later life, when it can lead to potentially significant improvements to your quality of life and your physical and mental wellbeing."

Because the team measured physical activity and sedentary behaviour at different points of time, they say they can be reasonably confident that they have shown a causal link -- that is, that quality of life improves becausepeople remain more physically active, for example.

Dr Yerrakalva added: "There are several ways in which improvements in our physical behaviours might help maintain a better quality of life. For example, more physical activity reduces pain in common conditions such as osteoarthritis, and we know that being more physically active improves muscle strength which allows older adults to continue to care for themselves. Similarly, depression and anxiety are linked to quality of life, and can be improved by being more active and less sedentary."

The EPIC-Norfolk study is funded by the Medical Research Council and Cancer Research UK.

Fewer teens now perceive themselves as overweight -- international study of more than 745,000 adolescents

Changes in body weight perception could undermine public health interventions to tackle obesity

A study involving more than 745,000 adolescents from 41 countries across Europe and North America identified an increase in the amount of teenagers who underestimate their body weight.

Tracking data from 2002 to 2018, the peer-reviewed findings, published today in Child and Adolescent Obesity, demonstrate a noticeable decrease in those who overestimate their weight too.

The team of international experts, who carried out the research, warn these shifting trends in body weight perception could reduce the effectiveness of public health interventions aimed at weight reduction in young people.

"During this impressionable age, body weight perception may influence a young person's lifestyle choices, such as the amount and types of food they eat and their exercise habits," says lead author Doctor Anouk Geraets, from the Department of Social Sciences, at the University of Luxembourg.

"So it's concerning that we're seeing a trend where fewer adolescents perceive themselves as being overweight -- as this could undermine ongoing efforts to tackle increasing levels of obesity in this age group. Young people who underestimate their weight and therefore do not consider themselves to be overweight may not feel they need to lose excess weight and, as a result, they may make unhealthy lifestyle choices."

A person's perception of their body weight may not accurately reflect their actual weight. A discrepancy in body weight perception (BWP) may either be an underestimation (where actual weight is higher than perceived weight) or an overestimation (where actual weight is lower than perceived weight).

In the present study, the researchers examined survey data from 746,121 11-, 13- and 15-year-olds from 41 countries collected at four-yearly intervals between 2002 and 2018 in the International Health Behavior in School-Aged Children (HBSC), a WHO collaborative study.

The team modeled trends in BWP among adolescents across different countries over time, making adjustments for age, gender, and family socioeconomic status. They found:

  • Underestimation of weight status increased, and overestimation of weight status decreased over time among both sexes, with stronger trends for girls.

  • Correct weight perception increased over time among girls, while it decreased among boys.

  • Changes in correct weight perception, underestimation and overestimation of weight status differed across different countries -- but these changes could not be explained by an increase in country-level overweight/obesity prevalence.

The authors speculated that the observed differences between girls and boys in BWP may support the idea there are sex differences in body ideals -- and that these body ideals have changed over time. Notably, the increased underestimation and decreased overestimation of weight status over time for girls may be explained by the emergence of an athletic and strong body, as a new contemporary body ideal for both sexes.

"This study has clinical and public health implications. The increase in correct weight perception and the decrease in overestimation may have a positive effect on unnecessary and unhealthy weight loss behaviors among adolescents, while the increase in underestimation might indicate the need for interventions to strengthen correct weight perception," says lead author Doctor Anouk Geraets.

"More research is now needed to understand the factors underlying these time trends and to develop effective public health interventions."

While the large number of participating countries is a strength of the present study -- but as these only included countries in Europe, the USA and Canada, the results can't be generalized to other regions. In addition, although steps were taken to adjust the models for certain potential confounding factors, several other factors -- such as body image, dieting, changing eating patterns, or migration -- may also have played a role in the observed trends over time.

Getting adults on board with messy nature play

Climbing trees, making mud pies, or simply playing outside, parents and educators know that being in nature is an important part of every childhood. But when it comes to messy or risky play, it's a whole different story according to new research from the University of South Australia.

Talking to parents and early childhood educators, researchers found that parents and educators will happily open the door to 'safe' or 'clean' nature play but are more reluctant to let children engage in 'messy' activities or play that may be perceived as 'risky'.

UniSA researcher and PhD candidate Kylie Dankiw says that parents and educators act as important gatekeepers when it comes to nature play.

"Nature play is well known for its positive effects on children's health, development, and wellbeing," Dankiw says, "and this was a common theme when interviewing parents and carers alike.

"Nature play helps improve emotional regulation, physical skills, and learning outcomes, and can encourage children to develop their creativity and imagination.

"Parents and educators also identified that nature play can help children form a connection with the natural world and learn about sustainable practices.

"Importantly, parents and carers felt that nature play experiences could be used to offset technology use such as TV and screens.

"But despite the known benefits, parents and carers can find it difficult when it comes to engaging children in nature play, especially if activities are messy or dirty (such as water or mud play) or are thought of as being risky (such as climbing).

"Educators tell us that safety regulations and time restrictions can limit what they choose as outdoor activities, especially when they need to change children's clothes after muddy play, or when parents expect for their child to come home clean from childcare.

"There can be a conflict between encouraging children to experience nature, and what adults need to deal with in the so-called aftermath."

Nearly 50% of Australia's children aged 0-12 years (two million) spend time in formal or informal early childhood education care, with long day care being the most common type of care for children aged 0-4 years.

Paediatric expert UniSA's Dr Margarita Tsiros says given the large number of children in care, further education and training for both early childhood educators and parents could help overcome some of the challenges that might be linked with nature play.

"Our research highlights that opportunities for young children to engage in nature play is influenced by other people in their lives," Dr Tsiros says.

"While parents and educators recognise the benefits of nature play, they can struggle with certain aspects of being in nature, particularly risk.

"Understanding these barriers can help inform strategies to promote nature play for different age groups. It can also help inform policies and practices to promote enabling factors.

"A key move will be to boost educators' knowledge about nature-based learning, what constitutes nature play, and how they can use natural resources to facilitate nature play experiences.

"In a time where screens threaten to consume children's interest, it's vital that we present opportunities for them to engage in nature play, and to achieve this, we need to have parents and educators on board."

Combining maths with music leads to higher scores, suggests review of 50 years of research

Children do better at maths when music is a key part of their lessons, an analysis of almost 50 years of research on the topic has revealed.

It is thought that music can make maths more enjoyable, keep students engaged and help any ease fear or anxiety they have about maths. Motivation may be increased and pupils may appreciate maths more, the peer-reviewed article in Educational Studies details.

Techniques for integrating music into maths lessons range from clapping to pieces with different rhythms when learning numbers and fractions, to using maths to design musical instruments.

Previous research has shown that children who are better at music also do better at maths. But whether teaching music to youngsters actually improves their maths has been less clear.

To find out more, Turkish researcher Dr. Ayça Akin, from the Department of Software Engineering, Antalya Belek University, searched academic databases for research on the topic published between 1975 and 2022.

She then combined the results of 55 studies from around the world, involving almost 78,000 young people from kindergarten pupils to university students, to come up with an answer.

Three types of musical intervention were included the meta-analysis: standardised music interventions (typical music lessons, in which children sing and listen to, and compose, music), instrumental musical interventions (lessons in which children learn how to play instruments, either individually or as part of a band) and music-maths integrated interventions, in which music is integrated into maths lessons.

Students took maths tests before and after taking part in the intervention and the change in their scores was compared with that of youngsters who didn't take part in an intervention.

The use of music, whether in separate lessons or as part of maths classes, was associated with greater improvement in maths over time.

The integrated lessons had the biggest effect, with around 73% of students who had integrated lessons doing significantly better than youngsters who didn't have any type of musical intervention.

Some 69% of students who learned how to play instruments and 58% of students who had normal music lessons improved more than pupils with no musical intervention.

The results also indicate that music helps more with learning arithmetic than other types of maths and has a bigger impact on younger pupils and those learning more basic mathematical concepts.

Dr Akin, who carried out the research while at Turkey's National Ministry of Education and Antalya Belek University, points out that maths and music have much in common, such as the use of symbols symmetry. Both subjects also require abstract thought and quantitative reasoning.

Arithmetic may lend itself particularly well to being taught through music because core concepts, such as fractions and ratios, are also fundamental to music. For example, musical notes of different lengths can be represented as fractions and added together to create several bars of music.

Integrated lessons may be especially effective because they allow pupils to build connections between the maths and music and provide extra opportunities to explore, interpret and understand maths.

Plus, if they are more enjoyable than traditional maths lessons, any anxiety students feel about maths may be eased.

Limitations of the analysis include the relatively small number of studies available for inclusion. This meant it wasn't possible to look at the effect of factors such as gender, socio-economic status and length of musical instruction on the results.

Dr Akin, who is now based at Antalya Belek University, concludes that while musical instruction overall has a small to moderate effect on achievement in maths, integrated lessons have a large impact.

She adds: "Encouraging mathematics and music teachers to plan lessons together could help ease students' anxiety about mathematics, while also boosting achievement."

Fasting can help you lose weight, but you might gain it back quickly

Water fasts -- where people consume nothing but water for several days -- might help you lose weight, but it's unclear how long you'll keep it off, according to research from the University of Illinois Chicago. And the other metabolic benefits of water fasts, such as lower blood pressure and improved cholesterol, seem to disappear soon after the fast ends, the researchers found.

However, there do not appear to be any serious adverse effects for those who do a water fast or a similar kind of fast where people consume a very small number of calories a day, said Krista Varady, professor of kinesiology and nutrition, who led the research, which is published in Nutrition Reviews.

"My overall conclusion is that I guess you could try it, but it just seems like a lot of work, and all those metabolic benefits disappear," Varady said. She stressed, however, that no one should undertake one of these fasts for more than five days without medical supervision.

Varady, an expert on intermittent fasting, said she wanted to study water fasting because she suddenly started getting contacted by journalists last fall who wanted to hear what she thought about it. She figured if she was going to comment, she should investigate the existing research.

The new paper is a literature review of eight studies on water fasting or Buchinger fasting, a medically supervised fast that is popular in Europe where people consume only a tiny amount of juice and soup a day. Varady's team looked at the results from each of those papers to see what story they cumulatively tell about the fasts' impact on weight loss, as well as a number of other metabolic factors.

Fasting did seem to spur noticeable short-term weight loss, the researchers found. People who fasted for five days lost about 4% to 6% of their weight; those who fasted for seven to 10 days lost about 2% to 10%, and those who fasted for 15 to 20 days lost 7% to 10%.

Only a few of the studies in the review tracked whether participants gained back the weight they had lost once the fast ended. In one of those, people gained back all they had lost in a five-day water fast within three months. In two other studies, only a small amount of the lost weight returned, but those studies encouraged participants to restrict their calorie intake after the fasts ended.

In contrast, it was clear that the metabolic benefits of the fasts disappeared soon after the fasts ended. Improvements to blood pressure, cholesterol and blood sugar levels were short-lived, returning to baseline levels quickly after participants started eating again.

Some of the studies included participants with Type 1 and Type 2 diabetes, who suffered no ill effects from the fasting, though they were monitored closely and had their insulin doses adjusted while fasting.

The most common side effects of these prolonged fasts were similar to those from intermittent fasting, Varady said, such as headaches, insomnia and hunger. There were no serious negative effects in the studies, such as metabolic acidosis or death.

She did note that the participants in these prolonged fasts lost about two-thirds of their weight in lean mass and one-third in fat mass. This is the opposite of what happens most of the time during weight loss, where more fat is lost than muscle. It makes sense that these extreme fasts would have this result, she said, because "your body needs a constant intake of protein. If it doesn't have that, then it draws from muscles."

Varady's research into intermittent fasting has looked at how well the regime works for weight loss, as well as at specific questions, such as whether intermittent fasting affects fertility -- she found it does not.

Varady said she would encourage someone hoping to lose weight to try intermittent fasting instead of a water fast, "because there's a lot more data to show it can help with weight management," she said.

Taking higher-than-recommended doses of vitamin D for five years reduced the risk of atrial fibrillation

Taking higher-than-recommended doses of vitamin D for five years reduced the risk of atrial fibrillation in older men and women, according to a new study from the University of Eastern Finland.

Atrial fibrillation is the most common arrhythmia, the risk of which increases with age, and which is associated with an increased risk of stroke, heart failure and mortality. Vitamin D has been shown to have an effect, for example, on the atrial structure and the electrical function of the heart, suggesting that vitamin D might prevent atrial fibrillation.

Conducted at the University of Eastern Finland in 2012-2018, the main objective of the Finnish Vitamin D Trial, FIND, was to explore the associations of vitamin D supplementation with the incidence of cardiovascular diseases and cancers. The five-year study involved 2,495 participants, 60-year-old or older men and 65-year-old or older women, who were randomised into three groups: one placebo group and two vitamin D3 supplementation groups, with one of the groups taking a supplement of 40 micrograms (1600 IU) per day, and the other a supplement of 80 micrograms (3200 IU) per day. All participants were also allowed to take their personal vitamin D supplement, up to 20 micrograms (800 IU) per day, which at the beginning of the study was the recommended dose for this age group. At baseline, study participants had not been diagnosed with cardiovascular disease or cancer, and they completed comprehensive questionnaires, both at the beginning and throughout the study, on their lifestyles and nutrition, as well as on risk factors of diseases and disease occurrence. Data on the occurrence of diseases and deaths were also obtained from Finnish nationwide health registers. Approximately 20 % of participants were randomly selected for more detailed examinations and blood samples.

During the five-year study, 190 participants were diagnosed with atrial fibrillation: 76 in the placebo group, 59 in the 40 micrograms group, and 55 in the 80 micrograms group. The risk of atrial fibrillation was 27% lower in the 40 micrograms group, and 32% lower in the 80 micrograms group, when compared to the placebo group. In the sub-cohort selected for more detailed examinations, the mean baseline serum calcidiol concentration, which is a marker of the body's vitamin D concentration, was relatively high, 75 nmol/l. After one year, the mean calcidiol concentration was 100 nmol/l in the 40 micrograms group, and 120 nmol/l in the 80 micrograms group. No significant change in the calcidiol concentration was observed in the placebo group.

FIND is the first randomized controlled trial to observe that vitamin D supplementation reduces the risk of atrial fibrillation in generally healthy men and women. Previous research is limited to only two randomized trials, which did not observe an effect when using doses of 10 micrograms (400 IU) or 50 micrograms (2000 IU) per day. Further confirmation of the present results from the FIND study is therefore needed before doses of vitamin D that significantly exceed current recommendations can be recommended for preventing atrial fibrillation. The FIND study has previously published findings showing no association with the incidence of other cardiovascular events or cancers.

Reading for pleasure early in childhood linked to better cognitive performance and mental wellbeing in adolescence

Children who begin reading for pleasure early in life tend to perform better at cognitive tests and have better mental health when they enter adolescence, a study of more than 10,000 young adolescents in the US has found.

In a study published today in Psychological Medicine, researchers in the UK and China found that 12 hours a week was the optimal amount of reading, and that this was linked to improved brain structure, which may help explain the findings.

Reading for pleasure can be an important and enjoyable childhood activity. Unlike listening and spoken language, which develop rapidly and easily in young children, reading is a taught skill and is acquired and developed through explicit learning over time.

During childhood and adolescence, our brains develop, making this an important time in which to establish behaviours that support our cognitive development and promote good brain health.  However, until now it has been unclear what impact – if any – encouraging children to read from an early age will have on their brain development, cognition and mental health later in life.

To investigate this, researchers from the universities of Cambridge and Warwick in the UK and Fudan University in China looked at data from the Adolescent Brain and Cognitive Development (ABCD) cohort in the US, which recruited more than 10,000 young adolescents.

The team analysed a wide range of data including from clinical interviews, cognitive tests, mental and behavioural assessments and brain scans, comparing young people who began reading for pleasure at a relatively early age (between two and nine years old) against those who began doing so later or not at all. The analyses controlled for many important factors, including socio-economic status.

Of the 10,243 participants studied, just under a half (48%) had little experience of reading for pleasure or did not begin doing so until later in their childhood. The remaining half had spent between three and ten years reading for pleasure.

The team found a strong link between reading for pleasure at an early age and a positive performance in adolescence on cognitive tests that measured such factors as verbal learning, memory and speech development, and at school academic achievement.

These children also had better mental wellbeing, as assessed using a number of clinical scores and reports from parents and teachers, showing fewer signs of stress and depression, as well as improved attention and fewer behavioural problems such as aggression and rule-breaking.

Children who began reading for pleasure earlier also tended to spend less screen time – for example watching TV or using their smartphone or tablet – during the week and at weekends in their adolescence, and also tended to sleep longer.

When the researchers looked at brain scans from the adolescent cohort, they found that those participants who had taken to reading for pleasure at an early age showed moderately larger total brain areas and volumes, including in particular brain regions that play critical roles in cognitive functions. Other brain regions that were different among this group were those that have been previously shown to relate to improved mental health, behaviour and attention.

Professor Barbara Sahakian from the Department of Psychiatry at the University of Cambridge said: “Reading isn’t just a pleasurable experience – it’s widely accepted that it inspires thinking and creativity, increases empathy and reduces stress. But on top of this, we found significant evidence that it’s linked to important developmental factors in children, improving their cognition, mental health, and brain structure, which are cornerstones for future learning and well-being.”

The optimal amount of reading for pleasure as a young child was around 12 hours per week. Beyond this, there appeared to be no additional benefits. In fact, there was a gradual decrease in cognition, which the researchers say may be because it suggests they are spending more time sedentary and less time at other activities that could be cognitively enriching, including sports and social activities.

Professor Jianfeng Feng from Fudan University in Shanghai, China, and the University of Warwick, UK, said: “We encourage parents to do their best to awaken the joy of reading in their children at an early age. Done right, this will not only give them pleasure and enjoyment, but will also help their development and encourage long-term reading habits, which may also prove beneficial into adult life.”

Poverty negatively impacts structural wiring in children's brains

Reducing obesity, boosting cognitive enrichment may improve kids' brain health

A new study from Washington University School of Medicine in St. Louis suggests that growing up in poverty may influence the wiring of a child's brain.

The study, published June 27 in JAMA Network Open, indicates a link between both neighborhood and household poverty and the brain's white matter tracts, which allow for communication between brain regions. White matter plays a critical role in helping the brain process information.

The findings stem from the largest long-term study of brain development and child health conducted in the U.S. -- the Adolescent Brain Cognitive Development (ABCD) Study, which was launched by the National Institutes of Health (NIH) in 2015. Washington University is a national leader in studies of the developing brain and is one of 21 study sites around the country participating in the ABCD Study, which is following nearly 12,000 children, beginning at ages 9 to 10, for at least a decade.

"White matter integrity is very important in brain development," said first author Zhaolong (Adrian) Li, a neuroimaging research technician in the Department of Psychiatry. "For example, weaknesses in white matter are linked to visuospatial and mental health challenges in children. If we can capture how socioeconomic status affects white matter early on in a child's life, the hope is we can, one day, translate these findings to preventive measures."

The researchers also found that childhood obesity and lower cognitive function may explain, at least partially, poverty's influence on white matter differences. Generally, children who grow up in poverty have a higher risk of obesity and score lower on tests of cognitive function than their peers in higher income neighborhoods and households. The latter could be due, in part, to limited access to enriching sensory, social and cognitive stimulation.

"Our finding that obesity and cognitive enrichment may be relevant mediators, if confirmed, would provide strong support for managing healthy weight and encouraging cognitively stimulating activities to support brain health in disadvantaged children," said Tamara Hershey, PhD, the James S. McDonnell Professor of Cognitive Neuroscience and a professor of psychiatry and of radiology.

The research was conducted in the Neuroimaging Labs Research Center in the university's Mallinckrodt Institute of Radiology.

White matter, the densely packed nerve fibers deep in the brain, gets its white color from the fatty substance that surrounds nerve fibers. The fatty coating is responsible for the rapid transmission of information along nerve cell tracts. The organization and connectivity between these tracts support learning and proper communication across brain regions. Disruption in these communication pathways has been linked to physical challenges as well as worse mental health outcomes.

The scientists used the publicly available ABCD Study database, through which they were able to model water movement as an indicator of white matter integrity in the brain scans of 8,842 children ages 9 to 11. Just like rocks, pebbles and boulders impact the flow of water in a river, diverse brain cell structures create barriers that hinder water diffusion. The researchers found less directional movement of water molecules in the brains of children living in poverty, signifying structural changes in white matter regions. They also found higher water content in spherical spaces in the brain, which hinted at possible neuroinflammation in children who live in poverty.

A child's environment is complex, involving both neighborhood and family influences. Disadvantaged neighborhoods suffer disproportionately from unemployment, poverty, and income disparity. Single-parent homes are more common, and residents are typically less educated, earn a lower income, and own less property.

"Our analysis revealed that neighborhood poverty is linked to white matter differences and putative immune cell presence. We found a similar link when looking at household socioeconomic status, taking into account annual income and parental education," Li said.

"Wealth and income inequality are accelerating in the U.S.," said co-corresponding author Scott Marek, PhD, an assistant professor of radiology and of psychiatry. "We and others are starting to scratch the surface of how inequality may harm the developing brain and affect mental health outcomes. Our findings emphasize shifting away from the thinking that socioeconomics is a unitary construct. It's not schools or parenting alone that matter for brain health. It's likely the collection of many neighborhood and familial life factors."

Hershey, who directs the Neuroimaging Labs Research Center and is a co-corresponding author, cautioned that the study only looked at one time point. Therefore, it is too soon to know if poverty triggered the brain differences seen in the study, she said. However, the ABCD Study continues to track enrolled children through brain scans and cognitive testing with the potential for future long-term brain development studies in disadvantaged children.

"We hope this work encourages future studies to examine modifiable health risk factors in large and longitudinal samples that would one day translate to intervention," Hershey said.

In a study that followed more than 2,000 community-dwelling older adults over eight years, researchers at Johns Hopkins Medicine say they have significant new evidence of a link between decreased sense of smell and risk of developing late-life depression.

Their findings, published June 26 in Journal of Gerontology: Medical Sciences, do not demonstrate that loss of smell causes depression, but suggests that it may serve as a potent indicator of overall health and well-being.

"We've seen repeatedly that a poor sense of smell can be an early warning sign of neurodegenerative diseases such as Alzheimer's disease and Parkinson's disease, as well as a mortality risk. This study underscores its association with depressive symptoms," says Vidya Kamath, Ph.D., associate professor of psychiatry and behavioral sciences at the Johns Hopkins University School of Medicine. "Additionally, this study explores factors that might influence the relationship between olfaction and depression, including poor cognition and inflammation."

The study used data gathered from 2,125 participants in a federal government study known as the Health, Aging and Body Composition Study (Health ABC). This cohort was composed of a group of healthy older adults ages 70-73 at the start of the eight-year study period in 1997-98. Participants showed no difficulties in walking 0.25 miles, climbing 10 steps or performing normal activities at the start of the study, and were assessed in person annually and by phone every six months. Tests included those for the ability to detect certain odors, depression and mobility assessments.

In 1999, when smell was first measured, 48% of participants displayed a normal sense of smell, 28% showed a decreased sense of smell, known as hyposmia, and 24% had a profound loss of the sense, known as anosmia. Participants with a better sense of smell tended to be younger than those reporting significant loss or hyposmia. Over follow-up, 25% of participants developed significant depressive symptoms. When analyzed further, researchers found that individuals with decreased or significant loss of smell had increased risk of developing significant depressive symptoms at longitudinal follow-up than those in the normal olfaction group. Participants with a better sense of smell tended to be younger than those reporting significant loss or hyposomia.

Researchers also identified three depressive symptom "trajectories" in the study group: stable low, stable moderate and stable high depressive symptoms. Poorer sense of smell was associated with an increased chance of a participant falling into the moderate or high depressive symptoms groups, meaning that the worse a person's sense of smell, the higher their depressive symptoms. These findings persisted after adjusting for age, income, lifestyle, health factors and use of antidepressant medication.

"Losing your sense of smell influences many aspects of our health and behavior, such as sensing spoiled food or noxious gas, and eating enjoyment. Now we can see that it may also be an important vulnerability indicator of something in your health gone awry," says Kamath. "Smell is an important way to engage with the world around us, and this study shows it may be a warning sign for late-life depression."

Humans' sense of smell is one of two chemical senses. It works through specialized sensory cells, called olfactory neurons, which are found in the nose. These neurons have one odor receptor; it picks up molecules released by substances around us, which are then relayed to the brain for interpretation. The higher the concentration of these smell molecules the stronger the smell, and different combinations of molecules result in different sensations.

Smell is processed in the brain's olfactory bulb, which is believed to interact closely with the amygdala, hippocampus and other brain structures that regulate and enable memory, decision-making and emotional responses.

The Johns Hopkins researchers say their study suggests that olfaction and depression may be linked through both biological (e.g., altered serotonin levels, brain volume changes) and behavioral (e.g., reduced social function and appetite) mechanisms.

The researchers plan to replicate their findings from this study in more groups of older adults, and examine changes to individuals' olfactory bulbs to determine if this system is in fact altered in those diagnosed with depression. They also plan to examine if smell can be used in intervention strategies to mitigate risk of late-life depression.

Fighting loneliness by finding purpose

A sense of purpose in life -- whether it's a high-minded quest to make a difference or a simple hobby with personal meaning -- can offer potent protection against loneliness, according to new research.

A new study co-authored by Patrick Hill, associate professor of psychological and brain sciences, offers an important message for our times: A sense of purpose in life -- whether it's a high-minded quest to make a difference or a simple hobby with personal meaning -- can offer potent protection against loneliness.

"Loneliness is known to be one of the biggest psychological predictors for health problems, cognitive decline, and early mortality," Hill said. "Studies show that it can be as harmful for health as smoking or having a poor diet."

The new study, based on surveys of more than 2,300 adults in Switzerland, found that feelings of loneliness were less common in people who reported a purposeful life, regardless of their age. It was co-authored by Mathias Allemand of the University of Zurich in Switzerland and Gabriel Olaru of Tilburg University in the Netherlands.

Respondents were asked to score their feelings on a lack of companionship, isolation from other people, and a sense of being "left out or passed over" during a four-week period. Participants also filled out the six-item Life Engagement Test, which asked them to rate statements such as "there is not enough purpose in my life" and "I value my activities a lot."

"A sense of purpose is this general perception that you have something leading and directing you from one day to the next," Hill said. "It can be something like gardening, supporting your family, or achieving success at work."

Many of the activities that can provide a sense of purpose -- joining a club, volunteering at a school, playing in a sports league -- involve interaction with others, which is one reason why a purpose-filled life tends to be less lonely. In the study, people who said they received or provided social support were especially likely to report feelings of purpose.

But Hill noted that there's more to fighting loneliness than simply being around others. "We've all had time in our lives when we've felt lonely even though we weren't actually alone." There's something about having a sense of purpose that seems to fight loneliness regardless of how many other people are involved, he said.

The study found a slight uptick in reports of loneliness for people in their 70s and beyond, an age when a sense of purpose can be especially important. "We're trying to dispel the myth from previous generations that this is simply a time for retiring and resting," Hill said. "There are no downsides to finding something meaningful later in life."

Still, it's important to keep in mind that a quest for purpose can be somewhat self-defeating if taken too seriously. "Feeling like you need to save the world can lead to existential dread and distress," Hill said. When it comes to purpose and meaning, even small things can matter. "It's OK if someone else thinks that your purpose is trivial, as long as it's meaningful to you."

Lean body mass, age linked with alcohol elimination rates in women

Research links women's lean body mass with how quickly they eliminate alcohol from their system. Women with obesity and those who are older eliminate alcohol from their bloodstreams faster than those of normal weight and those who are younger.

 

The rate at which women eliminate alcohol from their bloodstream is largely predicted by their lean body mass, although age plays a role, too, scientists found in a new study. Women with obesity -- and those who are older -- clear alcohol from their systems 52% faster than women of healthy weights and those who are younger, the study found.

Lean body mass is defined in the study -- published in the journal Alcohol Clinical and Experimental Research -- as one's total body weight minus fat.

"We believe the strong relationship we found between participants' lean body mass and their alcohol elimination rate is due to the association that exists between lean body mass and lean liver tissue -- the part of the liver responsible for metabolizing alcohol," said research group leader M. Yanina Pepino, a professor of food science and human nutrition at the University of Illinois Urbana-Champaign.

To explore links between body composition and alcohol elimination rates, the team conducted a secondary analysis of data from a study performed at the U. of I and another at Indiana University, Indianapolis. Both projects used similar methods to estimate the rate at which alcohol is broken down in the body.

The combined sample from the studies used in the analysis included 143 women who ranged in age from 21 to 64 and represented a wide range of body mass indices -- from healthy weights to severe obesity. Among these were 19 women who had undergone different types of bariatric surgery.

In a subsample of 102 of these women, the researchers had measured the proportions of lean and fat tissue in their bodies and calculated their body mass indices. Based on their BMI, those in the subsample were divided into three groups: normal weight, which included women with BMI ranging from 18.5-24.9; overweight, those with BMI ranging from 25-29.9; and obese, participants with BMI above 30.

As the researchers expected, women with higher BMI had not only more fat mass than women of healthy weights, they also had more lean mass. On average, the group with obesity had 52.3 kg of lean mass, compared with 47.5 kg for the normal weight group.

The two studies both used an alcohol clamp technique, where participants received an intravenous infusion of alcohol at a rate controlled by a computer-assisted system. The system calculated personalized infusion rates based upon each participant's age, height, weight and gender and was programmed so they would reach a target blood alcohol concentration of .06 percent within 15 minutes and maintain that level for about two hours

Using a breathalyzer, breath samples were collected at regular intervals throughout the experiments to estimate participants' blood alcohol concentration and provide feedback to the system.

"We found that having a higher fat-free body mass was associated with a faster alcohol elimination rate, particularly in women in the oldest subgroups," said Neda Seyedsadjadi, a postdoctoral fellow at the university and the first author of the study.

"The average alcohol elimination rates were 6 grams per hour for the healthy weight group, 7 grams for the overweight group, and 9 grams for the group with obesity," she said. "To put this in perspective, one standard drink is 14 grams of pure alcohol, which is found in 12 ounces of beer, 5 ounces of table wine or 1.5 ounces shot of distilled spirits."

The interaction between participants' age and lean body mass accounted for 72% of the variance in the time required to eliminate the alcohol from their system, the team found.

Pepino, who also holds an appointment as a health innovation professor at Carle Illinois College of Medicine, has conducted several studies on alcohol response in bariatric surgery patients.

The findings also shed light on alcohol metabolism and body composition in women who have undergone weight loss surgery. Researchers have long known that bariatric surgery alters women's response to alcohol but were uncertain if it affected how quickly they cleared alcohol from their systems.

Some prior studies found that these patients metabolized alcohol more slowly after they had weight loss surgery. The new study's findings indicate that these participants' slower alcohol elimination rates can be explained by surgery-induced reductions in their lean body mass. Weight loss surgery itself had no independent effects on patients' alcohol elimination rates, the team found.

Unraveling the connections between the brain and gut

MIT engineers' new technology can probe the neural circuits that influence hunger, mood, and a variety of diseases

Engineers designed a technology to probe connections between the brain and the digestive tract. Using fibers embedded with a variety of sensors, as well as optogenetic stimulation, the researchers could control neural circuits connecting the gut and the brain, in mice.

The brain and the digestive tract are in constant communication, relaying signals that help to control feeding and other behaviors. This extensive communication network also influences our mental state and has been implicated in many neurological disorders.

MIT engineers have now designed a new technology that can be used to probe those connections. Using fibers embedded with a variety of sensors, as well as light sources for optogenetic stimulation, the researchers have shown that they can control neural circuits connecting the gut and the brain, in mice.

In a new study, the researchers demonstrated that they could induce feelings of fullness or reward-seeking behavior in mice by manipulating cells of the intestine. In future work, they hope to explore some of the correlations that have been observed between digestive health and neurological conditions such as autism and Parkinson's disease.

"The exciting thing here is that we now have technology that can drive gut function and behaviors such as feeding. More importantly, we have the ability to start accessing the crosstalk between the gut and the brain with the millisecond precision of optogenetics, and we can do it in behaving animals," says Polina Anikeeva, the Matoula S. Salapatas Professor in Materials Science and Engineering, a professor of brain and cognitive sciences, associate director of MIT's Research Laboratory of Electronics, and a member of MIT's McGovern Institute for Brain Research.

Anikeeva is the senior author of the new study, which appears today in Nature Biotechnology. The paper's lead authors are MIT graduate student Atharva Sahasrabudhe, Duke University postdoc Laura Rupprecht, MIT postdoc Sirma Orguc, and former MIT postdoc Tural Khudiyev.

The brain-body connection

Last year, the McGovern Institute launched the K. Lisa Yang Brain-Body Center to study the interplay between the brain and other organs of the body. Research at the center focuses on illuminating how these interactions help to shape behavior and overall health, with a goal of developing future therapies for a variety of diseases.

"There's continuous, bidirectional crosstalk between the body and the brain," Anikeeva says. "For a long time, we thought that the brain is a tyrant that sends output into the organs and controls everything. But now we know that there's a lot of feedback back into the brain, and this feedback potentially controls some of the functions that we have previously attributed exclusively to the central neural control."

Anikeeva, who directs the new center, was interested in probing the signals that pass between the brain and the nervous system of the gut, also called the enteric nervous system. Sensory cells in the gut influence hunger and satiety via both the neuronal communication and hormone release.

Untangling those hormonal and neural effects has been difficult because there hasn't been a good way to rapidly measure the neuronal signals, which occur within milliseconds.

"To be able to perform gut optogenetics and then measure the effects on brain function and behavior, which requires millisecond precision, we needed a device that didn't exist. So, we decided to make it," says Sahasrabudhe, who led the development of the gut and brain probes.

The electronic interface that the researchers designed consists of flexible fibers that can carry out a variety of functions and can be inserted into the organs of interest. To create the fibers, Sahasrabudhe used a technique called thermal drawing, which allowed him to create polymer filaments, about as thin as a human hair, that can be embedded with electrodes and temperature sensors.

The filaments also carry microscale light-emitting devices that can be used to optogenetically stimulate cells, and microfluidic channels that can be used to deliver drugs.

The mechanical properties of the fibers can be tailored for use in different parts of the body. For the brain, the researchers created stiffer fibers that could be threaded deep into the brain. For digestive organs such as the intestine, they designed more delicate rubbery fibers that do not damage the lining of the organs but are still sturdy enough to withstand the harsh environment of the digestive tract.

"To study the interaction between the brain and the body, it is necessary to develop technologies that can interface with organs of interest as well as the brain at the same time, while recording physiological signals with high signal-to-noise ratio," Sahasrabudhe says. "We also need to be able to selectively stimulate different cell types in both organs in mice so that we can test their behaviors and perform causal analyses of these circuits."

The fibers are also designed so that they can be controlled wirelessly, using an external control circuit that can be temporarily affixed to the animal during an experiment. This wireless control circuit was developed by Orguc, a Schmidt Science Fellow, and Harrison Allen '20, MEng '22, who were co-advised between the Anikeeva lab and the lab of Anantha Chandrakasan, dean of MIT's School of Engineering and the Vannevar Bush Professor of Electrical Engineering and Computer Science.

Driving behavior

Using this interface, the researchers performed a series of experiments to show that they could influence behavior through manipulation of the gut as well as the brain.

First, they used the fibers to deliver optogenetic stimulation to a part of the brain called the ventral tegmental area (VTA), which releases dopamine. They placed mice in a cage with three chambers, and when the mice entered one particular chamber, the researchers activated the dopamine neurons. The resulting dopamine burst made the mice more likely to return to that chamber in search of the dopamine reward.

Then, the researchers tried to see if they could also induce that reward-seeking behavior by influencing the gut. To do that, they used fibers in the gut to release sucrose, which also activated dopamine release in the brain and prompted the animals to seek out the chamber they were in when sucrose was delivered.

Next, working with colleagues from Duke University, the researchers found they could induce the same reward-seeking behavior by skipping the sucrose and optogenetically stimulating nerve endings in the gut that provide input to the vagus nerve, which controls digestion and other bodily functions.

"Again, we got this place preference behavior that people have previously seen with stimulation in the brain, but now we are not touching the brain. We are just stimulating the gut, and we are observing control of central function from the periphery," Anikeeva says.

Sahasrabudhe worked closely with Rupprecht, a postdoc in Professor Diego Bohorquez' group at Duke, to test the fibers' ability to control feeding behaviors. They found that the devices could optogenetically stimulate cells that produce cholecystokinin, a hormone that promotes satiety. When this hormone release was activated, the animals' appetites were suppressed, even though they had been fasting for several hours. The researchers also demonstrated a similar effect when they stimulated cells that produce a peptide called PYY, which normally curbs appetite after very rich foods are consumed.

The researchers now plan to use this interface to study neurological conditions that are believed to have a gut-brain connection. For instance, studies have shown that children with autism are far more likely than their peers to be diagnosed with GI dysfunction, while anxiety and irritable bowel syndrome share genetic risks.

"We can now begin asking, are those coincidences, or is there a connection between the gut and the brain? And maybe there is an opportunity for us to tap into those gut-brain circuits to begin managing some of those conditions by manipulating the peripheral circuits in a way that does not directly 'touch' the brain and is less invasive," Anikeeva says.

Is TBI a chronic condition?

Study finds function may improve, decline up to seven years after injury

People with TBI may continue to improve or decline years after their injury, making it a more chronic illness, according to a study published in the June 21, 2023, online issue of Neurology®, the medical journal of the American Academy of Neurology.

"Our results dispute the notion that TBI is a one-time event with a stagnant outcome after a short period of recovery," said study author Benjamin L. Brett, PhD, of the Medical College of Wisconsin in Milwaukee. "Rather, people with TBI continue to show improvement and decline across a range of areas including their ability to function and their thinking skills."

The study involved people at 18 level 1 trauma center hospitals with an average age of 41. A total of 917 people had mild TBI and 193 people had moderate to severe TBI. They were matched to 154 people with orthopedic injuries but no head injuries. Participants were followed for up to seven years.

Participants took three tests on thinking, memory, mental health and ability to function with daily activities annually from two to seven years post-injury. They also completed an interview on their abilities and symptoms, including headache, fatigue, and sleep disturbances.

When researchers looked at all test scores combined, 21% of people with mild TBI experienced decline, compared to 26% of people with moderate to severe TBI and 15% of people with orthopedic injuries with no head injury.

Among the three tests, researchers saw the most decline over the years in the ability to function with daily activities. On average, over the course of 2 to 7 years post-injury, a total of 29% of those with mild TBI declined in their abilities and 23% of those with moderate to severe TBI.

Yet some people showed improvement in the same area, with 22% of those with mild TBI improving over time and 36% of those with moderate to severe TBI.

"These findings point out the need to recognize TBI as a chronic condition in order to establish adequate care that supports the evolving needs of people with this condition," Brett said. "This type of care should place a greater emphasis on helping people who have shown improvement continue to improve and implementing greater levels of support for those who have shown decline."

A limitation of the study was that all participants were seen at a level 1 trauma center hospital within 24 hours of their injury, so the findings may not apply to other populations.

The study was funded by the National Institute of Neurological Disorders and Stroke, National Institute on Aging, the National Football League Scientific Advisory Board and the U.S. Department of Defense.
